Abstract: At present, the prevalence of diabetes is on the rise, while the cardiovascular disease is a major complication of diabetes mellitus. Acute coronary syndrome (ACS) is a severe form of coronary heart disease. Compared with nondiabetic patients, there are more difficulties and troubles in deal with diabetic ACS patients with more serious condition. Although percutaneous coronary intervention (PCI) played a certain role on recanalization for coronary artery, the problems facing PCI are still serious for the diabetic ACS patients after the operation. Traditional Chinese medicine (TCM) confirms that the etiology and pathogenesis of diabetic ACS are deficiency in Qi and yin, and accumulation in stasis and toxin. Through clinical observation and retrospective analysis, We found that the “Supplementing Qi and nourishing Yin, activating blood circulation and detoxifying”, which simplified the treatment prescription, are integrated treatment of multiple links, multiple path ways, and multiple targets for the diabetic ACS, and are the unique advantages of the holistic concept, syndrome differentiation for treating and multi-levels intervention of TCM full practice in the comprehensive prevention and treatment for diabetic ACS.

Abstract: Aim: To study whether cerebral glucose metabolic disorder in malignant tumor patients without cerebral diseases is related to the site of tumor by using the 18F-deoxyglucose (FDG) PET/CT brain imaging technology. Methods: 22 patients with primary liver cancer and 20 patients with pancreatic cancer were subjected to general physical examinations by 18F-FDG positron emission tomography (PET). A statistical parametric mapping (SPM) software was adopted to analyze the information about cerebral resting glucose metabolism retrospectively, and to compare with the 22 healthy subjects with matched ages and genders. Results: Both the primary liver cancer and pancreatic cancer patients underwent metabolic reduction in both sides of the frontal and temporal areas, but the range and voxel involved in the latter were more extensive than those in the former, especially in the prefrontal cortex. The elevated metabolic areas in pancreatic cancer, which were much wider than those in liver cancer, were mainly located on both sides of the hippocampus, parahippocampal gyrus, amygdala, anterior cingulate cortex and posterior cingulate cortex, and extensively increased glucose metabolism was discerned in cerebellum. The areas of the latter were mainly distributed in the posterior cingulate cortex. Conclusion: Wide areas of glucose metabolic disorder existed in the patients with malignant tumors without cerebral metastasis, the distribution of which is related to the tumor type.

Abstract: Aim: The HFMD caused by EV71 virus has extremely high lethality and morbidity. EV71's pathogenesis has not understood thoroughly and the antiviral drug has been under development, which led to its emergence as a clinically fatal neurotropic virus. Early diagnosis is one of the main methods to reduce mortality. The aim of this study was to establish and evaluate a colloidal gold immunochromatography for qualitatively detecting the EV71-IgM in serum sample. Method: In this study, the different kinds of coating antigen were the focus of optimization. The pH value, particle size of the colloidal gold and other parameters and materials were also considered to achieve the good performance in detecting strips. The CA16-positive, EV71-positive and negative serum were used to compare and evaluate the accuracy and specificity of strips. Result: The accuracy of the strips coated by EV71-VP1-VP2-VP3 was higher than the EV71-VP1 strips 20%. The specificity of the EV71-VP1 strips was slightly higher than EV71-VP1-VP2-VP3 strips according to CA16 positive serum samples. Compared with commercial strips, this diagnostic method just needed 2 µL serum samples for detection, and the accuracy of the EV71-VP1-VP2-VP3 strips was significantly higher than the commercial strips. Conclusion: The developed method established in this study can apply to early diagnosis of HFMD caused by EV71 virus.

Surgery for Elder Patients with Colorectal Cancer and Risk Factors for Postoperative Complication

Abstract: With the increase in life expectancy and medical advance, surgical intervention for elderly patients with colorectal cancer is more and more frequent. In this retrospective study, this study mainly focused on surgery for elderly patients with colorectal cancer. In this study, the characteristics of preoperative, surgical and postoperative variables were described and risk factors for postoperative complication were analyzed. Totally, 221 patients were included in this study. The average age of patients was 78.84 years old and the preoperative nutritional status was roughly normal. 125 (56.56%) patients had comorbid diseases and 146 (67.59%) patients had an ASA score of 3 or 4. Emergency surgery was performed in 12 (5.43%) patients. 125 patients received open surgery while 96 received laparoscopic surgery among which 7 (7.29%) converted to open surgery. 36 (16.29%) patients had to get enterostomy. The average operative time was 220.00 min and the median blood loss was 100 ml with 55 (24.89%) patients needing intraoperative transfusion. As for risk factors for postoperative complication, patients with preoperative comorbid disease, longer operative time, more blood loss and intraoperative transfusion seemed more likely to have complication, although without significant difference. Preoperative low level of album (< 30 g/L) and emergency surgery were independent risk factors for postoperative complication (p = 0.037 and 0.021, respectively). In conclusion, surgery for elder patients with colorectal cancer had its own characteristics, and was controllable in general. Preoperative low level of album (< 30 g/L) and emergency surgery were independent risk factors for postoperative complication.
