Abstract: The editorial provides an overview of this special edition. The leading guest editors aim to guide the reader through the various topics addressed in this collection of article papers related to ethics as presented by the individual authors. Topics are placed within the context of current available literature related to the philosophy of ethics, ethics education as well as ethics in practice. The value of focussing on personal growth and reflection as part of the ethical journey of discovery of self and others is highlighted and encouraged.

The Enigma of Ethical Responsiveness: A Philosophical Perspective

Abstract: This essay considers the enigma of ethical responsiveness from a Levinasian point of view. According to Levinas, the self is ontologically driven to persist in self-interest and therefore “naturally” inclined to consider other persons and all other forms of alterity as means to sustain itself in existence. How, then, is ethical action – which places the interest of the Other before that of the self – possible? The violent and indifferent world in which we live seems to lend credence to Levinas’s contention that ethical action is an unlikely occurrence. At the same time, instances of goodness, i.e. small acts of kindness, great acts of bravery, and even lifespans of serving those in need occur on a daily basis amidst the prevailing callousness. While Levinas’s ethical metaphysics offers us thought-provoking insights into the possibility of ethical responsiveness in a largely apathetic world, his thought is not to be understood as a toolkit that offers practicable guidelines that may assist agents to negotiate the quagmire of ethical dilemmas in the real world. This essay starts by offering the reader a brief overview of the three dominant schools of ethical thought that underpin the various approaches to addressing ethical decision-making in the concrete world, a world that requires decisive action. Before addressing Levinas’s perspective and his invaluable contribution to this discussion, it is shown why none of these three dominant schools succeed unproblematically to provide practicable answers to ethical dilemmas.

Abstract: The complexities of modern healthcare pose a range of ethical issues for health professionals. In preparing student health professionals for these complexities, entry-level education must go beyond exposure to ethical theory, codes and standards. The aim of this systematic review was to determine the student learning outcomes reported in peer-reviewed allied health literature following ethics training. The review method included an a priori protocol, adherence to PRISMA statement, with critical appraisal via the Critical Appraisal Checklist for an Article on an Educational Intervention and The Critical Appraisal Skills Program (CASP) Qualitative Checklist. From a total of 1237 possible records published between 1991 and 2015, 11 studies of variable methodological quality met the inclusion criteria for final synthesis. Studies focused on teaching to occupational therapy, physiotherapy, pharmacy and dietetics students (n=588). Heterogeneity among studies precluded meta-analysis and data were synthesised qualitatively in a narrative format. A range of outcomes measures were used in the reviewed publications including the Structure of the Observed Learning Outcomes (SOLO), Defining Issues Test (DIT), Readiness for Interprofessional Learning Scale (RIPLS), Attitudes to Health Professionals Questionnaire (AHPQ), Student Development Task Lifestyle Inventory (SDTLI) and the Socio-moral Reflection Measure-short form (SRM-SF). Other studies surveyed students at the completion of the educational intervention using free text questions or Likert scales. The pedagogical approach used was varied, including transformative, constructivist, arts-based, inter-professional, problem-based or experiential learning approaches. The findings of this review suggest that given the current body of evidence, the teaching of ethics to and for allied health students should include the delivery of ethics focused content, be intensive in nature, link theory with real-life situations and practice, and expose students to multiple views using a variety of methods such as reflection and discussion of ambiguous, controversial topics. While the exact prescription of ethics training to maximise efficacy and uptake is unknown, there is some suggestion that immediate learning outcomes may follow a dose-response pattern, providing potential for future teaching models to incorporate repeated ethics exposure support for cumulative and sustained learning.

Abstract: As individuals engage in research and/or clinical practice, reference to, and guidance from, their professional codes of ethics permeates their work. There has been a rise in the number of web sites, workshops, and publications about ethical training and behavior. Increasingly, courses have been developed for students at the graduate level along with up to date content provided in the form of continuing education activity for professionals in practice. Contemporary approaches to ethics training have moved away from rule-governed practices to developing ethical decision frameworks. In addition, recognizing the character of the professional has become essential. Acknowledging the influences on developing students’ professional identities is fundamental to their graduate training. The purpose of this article is to illustrate one such approach to graduate training in ethics.

A Measuring Instrument for Ethical Sensitivity in the Therapeutic Sciences

Abstract: An emerging literature in behavioural ethics conceptualized ethical sensitivity as a critical part of the decision making process. Ethical sensitivity together with an understanding of the client, their needs, emotions and circumstances is fundamental to an effective therapeutic relationship and competent practice. This study appears to be the first to empirically measure this concept in decision making related to the therapeutic sciences, including audiology, occupational therapy, physiotherapy and speech-language therapy. A multidisciplinary measure of ethical sensitivity is developed and consists of 12 vignettes that represent clinically relevant ethical issues related to these four professions. The study followed a two-phase, sequential mixed-methods research approach. Phase 1, the qualitative stage, focused on developing a measuring instrument by means of a systematic review of the following: ethical codes of conduct; focus group discussions; individual in-depth interviews; an expert panel review; and public complaints websites. Phase 2, the quantitative stage, focused on implementing and evaluating the measuring instrument. One hundred participants representing the four professions completed the instrument. Participants’ overall scores on the Measuring Instrument for Ethical Sensitivity in the Therapeutic sciences (MIEST) were comparable for all four professions, confirming the multidisciplinary usability of the instrument. Participants were inclined to make grounded Beneficence centred decisions. Participants were particularly sensitive about the impact of the therapist’s actions on the individual client, and sometimes overlooked their duty to the community. The MIEST can be used to assess the ethical sensitivity of student therapists (and possibly qualified therapists) and describe the stage of their ethical sensitivity development throughout the course of their professional development. The constructed vignettes make the MIEST appropriate for use in problem-based learning programmes.

Abstract: Emmanuel Levinas [1] argued that ethics cannot be regarded as a set of principles, rules or norms, but rather that the fundamental basis of ethics is communication and negotiated decision-making. This article explores the conditions necessary for ethical behavior to occur, as explored by Murray [2], by focusing on two relevant issues: firstly the ability of the individual to announce themselves, i. e. their ability to open up or share of themselves, and secondly, their ability to represent themselves in issues surrounding decision-making. These two issues will be explored by referring to people with severe dementia and those who have severe communication problems. The paper includes the voice of the individual as well as the voice of those who interact with individuals with severe communication problems.

Abstract: Cultural competence and ethical decision making are two separate, yet intrinsically related concepts which are central to services rendered by all health care professionals. Cultural competence is based on ethical principles and informs ethical decision making. In spite of this important connection, the interrelationship of these two concepts does not receive the attention it deserves in the literature. This issue is addressed by appraising the training and assessment of cultural competence and ethical decision making in the health care professions. The integrated relationship of these two concepts is illustrated within the broader contexts of higher education, research and clinical practice. Health care professionals who incorporate cultural competence and ethical decision making will be empowered to provide the best services to their clients/ patients in multicultural contexts to ensure optimum outcomes.

Abstract: This paper traces the history of conducting research involving people with disabilities and raises serious questions about the ethics of conducting research. Ethical concerns include treatment of vulnerable populations, lack of informed consent, and benefit versus undue hardship when using people with disabilities as research participants. New technological advances, such as discussions on social media, present some new ethical concerns. Two case studies are presented that illustrate new ethical challenges. Guidelines are proposed that address the right to privacy, anonymity and confidentiality.
