Abstract: Arterial Blood Gases interpretation are considered as one of the significant investigations in the critical care areas that guides in appropriate direction to make better decision for diagnosis, treatment and expected outcome of the concerned disorders. Nurses are considered as the primary care providers in critical care units therefore their knowledge regarding ABGs interpretation and analysis may play a vital role in the management of the seriously ill patient admitted with respiratory, cardiovascular, acid base disorder and renal disease. The main intention of the study was to appraise the Effectiveness of the Instructional Module on Knowledge and Interpretation of ABGs among Critical Care Nurses. A quantitative quasi-experimental study design was used with a sample of 30 CCN by census method from Ayub Teaching hospital Abbottabad. Data was collected through a reliable and validated questionnaire before and after assigning with the instructional module. Data was analyzed by SPSS-22. The study result showed that the pretest knowledge score of the critical care nurses in the pre stage intervention was 53.3% which was poor on the grading scale. Overall mean score was found as 51.9963+26.54 in the pre assessment phase while 74.6333+15.98 was reported in the post phase of giving the instructional module with a mean significant difference of 22.63+39.50. Statistically the instructional module was found effective in enhancing the nurse’s knowledge with a –value of 0.004. The findings of the study depict that instructional module has a positive and significant association with the level knowledge of nurses regarding ABGs interpretation.

Abstract: The acceptance rate of hemodialysis (HD) in end stage renal disease (ESRD) patients has been increasing globally. Pakistan is a CKD high burden country with the increasing acceptance rate of HD. Unfortunately, there is dearth of published information regarding the rate, time to and factors associated with AVF maturation. The current retrospective cohort study was conducted with the aim to evaluate the rate, time to and factors associated with delayed AVF maturation. This study was conducted at the HD unit of Baluchistan Institute of Nephrology-Urology Quetta, Pakistan. The record of 120 HD patients who underwent native arteriovenous fistula (AVF) creation was reviewed focusing on the associated factors and arteriovenous fistula maturity. A standardized data collection form was used to abstract patients’ socio-demographic and clinical data. Delayed AVF maturation was defined as AVF which got mature after six weeks of its creation. Statistical Package for Social Sciences (SPSS version 20) was used for data analysis. Categorical data was presented at frequencies and percentages whereas, continuous data was presented as mean ± standard deviations. Chi-square and Fisher exact tests wherever applicable were conducted to find association between patients’ socio-demographic and clinical characteristics and delayed AVF maturation. A p-value <0.05 was considered statistically significant. Mean age and body mass index of the study participants were respectively 43.20 ±15.184 years and 20.14 ± 4.37 kg/m2. Majority of them were males (53.3%), belonged the age group 31-45 years (42.5%), rural residents (62.5%) and non-smokers (65.8%). A total of 93.3% patients were suffering from hypertension and 6.7% suffered from diabetes mellitus. Majority of the study participants had no family history of CKD (81.7%) and baseline visit of dialysis they had CKD duration of 2-5 years (47.5%). The mean time to AVF maturation in the current cohort was 41.9 ± 8.5 days. A total of 91.7% patients achieved the early maturation of AVF. No factor had statistical significant association with delayed AVF maturation. The rate of AVF maturation in the current cohort is comparatively high than reported in the published literature. Being a single centre study with retrospective design its results cannot be generalized. In the current study, 91.5% of the patients achieved early maturation of AVF. The rate of early maturation in the current cohort was comparatively higher than that reported in the previously published literature. In this retrospective study, we didn’t find out any factor which had significant association with time to AVF maturation.

Abstract: Background: Postnatal period is generally the most neglected period, especially in developing countries. This study assessed postnatal care service utilization and associated factors among mothers who gave birth in the last one year in Adama Town, Oromia, Ethiopia. Methods: Community based Cross sectional study was employed in seven randomly selected kebeles` in Adama town. A total of 563 mothers was included. Data was collected by face to face interview. Data were entered into Epi Info version 7 and imported to SPSS version 20. Descriptive statistics were carried out to characterize the study population. Bivariate and Multivariable Logistic regression were used to identify the associated factors. Result: The mean age of respondents was 28 (SD±5. 01) Years. The prevalence of PNC utilization was 269 (47.7%) with (95% CI: 44.0, 51.8). Educational status (AOR, 2.27; 95% CI: 1.13, 4.57), mode of delivery (AOR, 5.08; 95% CI: 2.55, 10.10), Provision of PNC appointment (AOR, 2.09; 95% CI: 1.36, 3.25), and Mothers Knowledge (AOR, 5.68; 95%CI: 2.97, 10.87) showed statistically significant association with PNC service utilization. Conclusion: Postnatal care service utilization in this study is much lower than Health service transformation plan of the country. Factors like Educational status, Mode of Delivery, Appointment date, mother’s knowledge of PNC utilization were found to be significantly associated with PNC utilization.

Abstract: Context: Moderate to severe pain has a profound negative impact on functioning and quality of life, hence nursing guidelines were needed as the driving force to optimize pain nursing interventions. Aim: To enhance the reduction of adult inpatients' pain intensity of not more than mild pain during the COVID-19 pandemic nursing care settings. Methods: For this hospital-wide pain management quality improvement initiative, quarterly cross-sectional pain prevalence surveys were conducted between February 2020 and February 2021. For patients with pain scores less than 4/10 only section one of the survey questionnaires was used, while those with pain scores more than mild pain chart review was performed utilizing section two of the questionnaire. The 80/20 principle was adopted to identify the area that needs attention for specific improvement actions of each unit that achieved less than 80% of patients with pain intensity less than 4/10. Results: University Hospital A achieved an average of 92% throughout the four quarters, and 87.4% was for University Hospital B. Less than 20% of patients during the four quarters of the surveys had moderate to severe pain in both hospitals. Conclusions: Both two hospitals achieved the target goal of more than 80% pain prevalence scores of not more than mild pain during the four quarters of the studies. It is therefore strongly recommended that healthcare organizations put in place policies and procedures that assign the nurses the target goal of effective pain management.
