Abstract: Background: Worldwide, breast cancer is the leading cause of cancer deaths among women. The practice of breast self-examination can heighten women awareness about the normal breast structure and composition, such option is recommended for early detection of breast cancer. The global shortage of medical workers has increased the call for involving and training community workers as rural pioneers to bridge such gap. The present study aimed to develop competency of rural women pioneers focused on early detection of breast cancer in New Valley governorate - Egypt. Subjects and Methods: A quasi-experimental design was used with a pre post-test. Purposive sample of 82 rural women pioneers in New Valley governorate was invited to take part in the study. Besides personal data, two tools were used to collect required data; a self-administered questionnaire sheet was prepared guided by Cancer Awareness Measure (CAM) and breast self-examination checklist adapted from the document prepared by Egypt Health Workforce Development Project. Results: More than half of the pioneers were at age group 30 to 40, with the mean age 39.1 ± 6.5 years, and 78% of them had more than 10 years of experience as rural pioneers. A statistically significant difference was found between study subjects' knowledge concerning cancer risk factors & warning signs pre-post intervention, and also in the practice of breast self-examination. Conclusion & recommendation: Nursing educational intervention designed for community manpower can successfully enhance breast cancer knowledge and practice. On the other hand, further research is recommended to replicate the study at different settings using larger samples to permit for generalization.

Abstract: Almost all patients receiving hepatitis C antiviral therapy will experience some treatment-related adverse effects. Emotional distress and fatigue can lead to early treatment discontinuation. Close monitoring is crucial throughout treatment. The nurses can promote adherence by counseling patients on the recognition and management of treatment- related adverse effects. The purpose of the study was to evaluate the effectiveness of nursing intervention program on emotional distress, self-efficacy and liver enzymes among hepatitis C Virus patients undergoing antiviral treatment therapy (Sovaldi medication). Quasi experimental design (one group pre/posttest) was used to achieve the aim of the study. The study was conducted in the Liver Institute at Shebin El kom District, Menoufia Governorate. A purposeful sample of 50 patients with hepatitis C virus patients undergoing antiviral treatment therapy (Sovaldi medication) from the above setting were recruited. Three instrument were used for data collection (1): Structured interview questionnaire to assess demographic and clinical data (2): Hospital anxiety and depression scale to measure emotional distress. (3): Self-efficacy scale to measure patients’ self-efficacy. The result revealed that there were statistical significant improvements of patient’s knowledge, patients’ emotional distress, liver enzymes and self-efficacy post program than preprogram. Conclusion: Nursing intervention program is the key element for management of patients’ emotional distress, self-efficacy and reduction of liver enzymes. Recommendation: psych educational program should be given for all patients with hepatitis C virus undergoing antiviral treatment therapy to improve their knowledge about the drug and promote adherence by counseling patients on the recognition and management of treatment related adverse effect. Encourage them to discuss their problem with health care team and participate in self-help program to decrease emotional distress and increase self-efficacy.

Abstract: Background: Sexuality is an important part of normal human functioning. Gynecological cancer diagnosis and treatment has a devastating effect on Sexual issues. The study was conducted at Outpatient oncology, Helwan General hospital, Egypt. The study aim to evaluate the effect of application of PLISSIT Counseling Model for women with cervical cancer undergoing treatment on enhancing sexuality. A quasi –experimental design was used. A purposive sample of 80 women with cervical cancer was included. Four tools, a structured interview questionnaire, body image scale, female sexual function index (FSDI), and the revised dyadic adjustment scale. The study findings indicated that a significant relationship was found between treatment, side effect pre and post application regarding nausea & vomiting, diarrhea, and pain. The study and research hypothesis concluded that applying of PLISSIT counseling model for sexuality was effective in enhancing sexual functioning except in the area of sexual desire or libido, improvement in couple satisfaction and body image after application of PLISSIT counseling model post application compared to pre-application. The study recommended that dissemination of PLISSIT Counseling Model For Women with cervical cancer in addressing sexual dysfunction in a cancer treatment, Applying the evidence-based nursing intervention to address and manage the effects of cervical cancer on sexuality.

Abstract: Social phobia (SP) is one of the most prevalent anxiety disorders among adolescent youth and remains under recognized and under treated. Environmental factors specifically parenting styles are particularly significant in the development of social phobia. Aim of the present study was to determine the association between social phobia and parenting styles among secondary school students. A descriptive design was selected in carrying out this study. The Setting of the present study was in the governmental schools in ElSinbellawin city, Dakhlia governerate. The sample size included a simple random sample composed of 446 students from three secondary schools; Ahmed Lofty El-Sayed secondary school for boys, El-Sadat secondary school for boys and The Secondary school for girls. Tools used for data collection were; sociodemographic data sheet, parenting styles and social phobia scales. Results revealed that female students recorded higher prevalence of social phobia (23.6%) than did in male students (14.4%). The significant difference was revealed between authoritarian, neglectful parenting styles and developing social phobia in the female students. There was a significant negative correlation between parenting styles and social phobia. This study concluded that social phobia is higher in females than in males. More the parents showed (authoritarian, authoritative, over-protective, neglectful) parenting style, their children showed lower levels of social phobia. The study recommended with early identification of socio phobic students by school nurse and social worker will be successfully help in providing early and adequate treatment, which in turn, reduce the burden of this common condition. Providing parents with educational programs about the efficacy of authoritative parenting style in nurturing their children.

Abstract: This study aimed to assess the quality of life of children (3-12 years) diagnosed with Thalassemia in Gaza Strip. A descriptive, analytical, cross-sectional design was used. All children 3-12 years old who were diagnosed with thalassemia and live in GS were included in the study (75 cases) with response rate (81.3%). Participants were interviewed by the researcher and completed the questionnaire which focused on quality of life of chronic diseases and included five domains. These domains are physical health, psychological health, social relationships, environment health and personal safety. Validity and reliability of the instrument were tested and the total instrument reliability test (Cronbach's Alpha) was 0.767. The scores for the study domains ranged between 2.81 for personal security domain and 3.66 for environmental health domain. Moreover, the study domains did not show statistically significant difference when compared by sex, living places, monthly income the relationship between parents and recurrent hospitalization. The findings of this study suggest the need of children diagnosed with thalassemia for support in all study domains. Health professionals need to work to minimize the disease's burdens. The current study calls the attention of health policy makers for new policies and new roles for the community health nurses and social workers. The study also revealed that there is a bad need to pay more attention when caring and dealing with thalassemia patients. Special food supplementation should be available in order to improve their physical health, more psychological support from their families and the society is needed. In addition, there is a need to provide safe environment and to enhance personal safety of these children.

Impact of Physical Stretching Exercise on Feto-Maternal Outcomes Among Mild Preeclamptic Pregnant Women in Egypt

Abstract: Preeclampsia is a disorder of widespread vascular endothelial malfunction and vasospasm that occurs after 20 weeks' gestation. It accounts for 22% of maternal deaths, 18% of all pre-mature births worldwide. Recently, recommendations about exercise in pregnant women with hypertension or at risk of pre-eclampsia (PE) development have been studied with the objective of trying to reduce the deleterious effects of hypertensive disorders in pregnancy, including the reduction of the incidence of PE. This study aimed to evaluate the impact of practicing self-independent physical stretching exercise by pregnant women with mild preeclampsia on their feto - maternal outcomes. Setting: The study was conducted in antenatal as well as labor units at El-Shatby Maternity University Hospital in Alexandria a purposive sample of 64 pregnant women participated in the study. Three tools were used to collect the necessary data and apply: Structured Interviewing Questionnaire to collect data regarding mothers socio-demographic variables, an observational check list which includes information related to fetomaternal outcomes and an instruction guideline which includes the importance and different types of stretching physical exercise. Results of the current study illustrated that the mean systolic and diastolic blood pressure before and after practice of stretching physical exercise had a significant difference (P=0.001). The practice of stretching physical exercise during pregnancy have ranged from not practice 100% before instruction to as high as 70.3% after interventions. As regards to fetomaternal outcomes, (55.6%) of studied subjects who practice exercise delivered vaginal and majority (89.7%) of them who not practice exercise delivered cesarean section and the reason for C.S.; fetal distress (52.6%). All (100%) of the study subject who practice exercise not suffer from complications while about one half (42.1%) who not practice exercise suffer from severe preeclampsia. physical exercise did not represent a risk of the neonatal outcomes studied: low birth weight (1500-2500 g) was 4.4%, adequacy of weight (95.6%), prematurity was (8.9%) and no birth defect was (89%). It was concluded that, practice of stretching physical exercise with mild preeclampsia promote mothers feto-maternal outcomes and not produce maternal or neonatal risk than who do not practice. The physical exercise was safe and it was not harmful to mother and newborn. So, it was recommended that low risk women should be adherence to physical exercise and lifestyle changes.
