Abstract: This paper aims to explore the influencing factors affecting the quality of life of older liver disease patients and the need for inpatient care, the nursing staff can more effectively help patients improve their quality of life. A total of 134 older patients with liver diseases were enrolled in our hospital from January 2018 to June 2019, and the general baseline data, quality of life (SF-36), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), family support, and care needs were surveyed. Univariate analysis showed SF-36 scores of older patients with liver diseases differed significantly (p<0.05) according to marital status (X1), course of disease (X2), disease diagnosis (X3), SAS (X4), SDS (X5) and family support intensity (X6) factors. Multiple linear regression analysis showed that marital status, disease diagnosis, SAS, SDS and family support were the influencing factors of quality of life of older patients with liver diseases. Nursing needs of older patients with liver diseases include disease care, psychological care needs that are associated with higher nursing needs, and lower life care needs. Marital status, disease diagnosis, SAS, SDS, and family support are the relevant factors influencing the quality of life of older patients with liver diseases. Therefore, nursing staff should provide effective nursing intervention in a timely manner according to the patient's condition and psychological needs.

Abstract: Objective: To investigate the effect of shared decision-making on compliance with oral nutrition supplementation (ONS) and quality of life of among elderly patients (≥ 60 years) with cirrhosis. Methods: This study used an action research design that tested the intervention of shared decision-making among elderly patients with cirrhosis. It measured how this intervention influenced compliance with oral nutrition supplements and quality of life. Sixty-eight elderly patients with cirrhosis who were hospitalized in our department from May 2019 to November 2019 were randomly divided into two groups: the control group the observation group. The total number of participants in each group was 34. The control group received routine care. The observation group received shared decision-making nutrition care in addition to routine care. Results: After the intervention, patients in the observation group had higher oral nutritional compliance and quality of life scores than the control group and had a statistical significance (P < 0.05). Conclusion: After the implementation of Shared decision nutrition care for 34 elderly patients with cirrhosis, the patients' ONS compliance and quality of life were greatly improved. At present, there are relatively few researches on the application of Shared decision in digestive system diseases,Hope to play a certain role of reference or guidance.

Abstract: Background: Clinical tutors play an important role and responsibility in the clinical training of nurses,They are indispensable members of clinical education training and the soul of continuing education training. Clinical interns from the school into the clinical work, Clinical mentors become leaders in their careers. In the training of new nurses, the one-to-one training method of clinical tutor is also more and more popular among hospitals at home and abroad. Subject: To investigate the psychological feelings of newly recruited nurses on the implementation of one-to-one specialized tutorial system training. It is hoped to provide references for the clinical teaching and training of new nurses. Methods: Semi-structured interviews were conducted with 6 nurses whom were employed as the newly recruited nurses, and the interview data were analysed and summarized. Results: Three recurring themes emerged: (1) stress at work (2) the sense of security and belonging (3) Create a professional image and increase professional passion (4) Comprehensive ability (5) Focus on teaching methods (6) a effective method. Conclusions: The one-to-one specialized tutorial system training will bring a sense of security and belonging to the newly recruited nurses, and it could also relieve their pressure when begin their career. The tutors play a positive role in training for professional quality, shaping for professional image and the love for the career, especially at an early development stage of career growth.

Abstract: Objective: To explore optimized processes and experience of cleaning operating room during continuous surgery. Method: Selected surgery cases from January to December 2018 randomly, 300 cases were using traditional methods of cleaning and 300 cases with improved methods of cleaning, we compared and analyzed the cleaning time and cleaning effect between two different methods, and investigated the changes of action ability of cleaning team, and analyzed the changes of doctor-patient satisfaction. Results: In 2018, we used improved cleaning management methods, cleaning time, cleaning effect, cleaning team action and patient or doctor satisfaction are significantly better than the traditional cleaning methods. Conclusion: In operating room, scientific cleaning management can significantly improve the efficiency of the operation, prevent the spread of bacteria in the operating room, improve the efficiency of the operating room, and thereby increase surgeon and patient satisfaction.

Abstract: Objective: To evaluate the influence of personalized nursing services on Severe acute pancreatitis patients who undergoing therapeutic plasma exchange. Methods: 62 patients were invited to join our research, that they were diagnosed as severe acute pancreatitis from March 2016 to October 2019. We randomly assigned the participants to control group (n = 31) and a intervention group (n = 31). On the hand, the control group participants have traditional nursing services. On the other hand, we provided the personalized nursing services to the intervention group participants. The patients report related information that include serum biochemical indicators, Vital signs change, acute physiology and chronic health evaluation (APACHE II) and multiple organ dysfunction syndrome (MODS). Result: Serum biochemical indices had great improvement was SCr (from 371.4±63.2 to 116.2±52.7 & from 879.3±139.4 to 395.7±135.4). Compare with control group and intervention, the intervention group patients had greater improvement in SCr and AMS, intervention group patients's improvement is about twice as great as control group patient’s. In vital signs change, the health status of patient is similar between control group and intervention group. In result of acute physiology and chronic health evaluation and multiple organ dysfunction syndrome, intervention group had better improvement in MODS domain, the influence of improvement is significant in the result. The MODS result of control group is worse than that of intervention group. Conclusion: The personalized nursing services provide great influence to reducing complications rate and treatment of speed. But the personalized nursing services only affect part of domains in the result, such as serum biochemical indices and MODS status.

Abstract: Objective: To assess humanistic care nursing mode for severe hyperbilirubinemia patients who undergoing double plasma molecular adsorption system and plasma exchange. Methods: 124 participants were diagnosed with severe hyperbilirubinemia and were received combined treatment, include that double plasma molecular adsorption system and Plasma exchange. They were randomly assigned to control group (n = 62) and intervention group (n = 62). We collect the participant information by Self-Rating Anxiety Scale, Self-rating depression scale and the Short Form-36. The information include the satisfaction of patients, anxiety status, depression status and quality of life. Result: In patient satisfaction research, the intervention group has more very good level assessment than that of control group (41 vs 32). Also, control group has more good level assessment than that of intervention group (18 vs 8). In anxiety and depression research, intervention group has better improvement in anxiety status, it is from 56.4±6.7 to 45.3±5.1. In quality of life research, the intervention group has greater improvement than that of control group in every domain. Conclusion: humanistic care nursing mode provide better improvement in mental health aspect of patient. It significantly improved anxiety and depression, and improved overall quality of life after treatment.

Abstract: Purpose: To explore the effect of continuous quality improvement (CQI) of radiation safety management for patients administerediodine-131 after thyroid cancer surgery. Methods: A total of 103 patients with differentiated thyroid carcinoma were randomly divided into control and experimental groups containing 51 and 52 patients, respectively. In the control group, the drug was administered according to the operating procedure for iodine-131 treatment of thyroid carcinoma. In the experimental group, CQI was adopted to manage the radiation safety of care in addition to the conventional iodine-131 thyroid cancer treatment procedures. We also improved radiation safety protection measures prior to drug administration, developed a flow chart of drug administration for patients, established a patient preview of the drug administration process, and enhanced health education and psychological intervention for the patient. Results: The environmental radiation around the drug delivery window was reduced (P< 0.05), total duration of exposure to the radiation was shortened (P< 0.05), drug drop rate was decreased to 0%, and patient satisfaction was improved in the experimental group compared to the control group. Conclusions: Application of CQI for management of radiation safety when treating thyroid cancer withiodine-131 can improve patient treatment, quality of care, and satisfaction and reduce radiation pollution in the surrounding environment and radiation injury to staff.

Abstract: Objective: To assess influence of individual care of case manager on breast cancer patients. Methods: 99 participants who were diagnosed as breast cancer were invested to join our study, their diagnose time is from October 2018 to March 2019. We randomly assign the participants to the two different groups, that include control group (n = 51) and the intervention group (n = 48). In control group, the participants received traditional care services which were follow the standards required by the hospital. Additionally, the intervention group participants receive individual care. our researchers collected the information, that include anxiety information, depression information, patient satisfaction and patient informed. We collected data using the following questionnaires: Self-Rating Anxiety Scale (SAS), Self-rating depression scale (SDS) and some simple questionnaire. Result: In participants characteristics, most participants are early breast cancer in our research result [22 (45.8%) & 25 (49.0%)]. The number of secondary is metaphase of breast cancer group, it is just a few fewer people than early breast cancer group [18 (37.5%) & 20 (39.2%)]. In research of satisfaction and informed intervention group has higher score than that of control group in satisfaction assessment and informed assessment. In anxiety status and depression status, the intervention group has better performance than that of control group in SAS and SDS (53.5+10.44 vs 41.5+8.82 & 49.75+9.45 vs 40.25+5.11). Conclusion: The individual care of case manager has good improvement in breast cancer patient’s mental health. In particular, the intervention group participants who receive individual care of case manager have better performance in satisfaction research, anxiety research and depression research.

Abstract: Objective To investigate the clinical effectiveness of intestinal cleaning knowledge training on intestinal cleaning rate of inpatients undergoing colonoscopy. Methods A comparative study was conducted by selection of 1166 inpatients undergoing colonoscopy in our hospital from January to November in 2016. Among them, 510 inpatients from January to June 2016 were induced into the pre-training group and 656 inpatients from July to December 2016 were induced into in the post-training group. The training contents included the indications and contraindications of colonoscopy, the dietary guidance before colonoscopy, the usage of laxatives for colonoscopy, the standards of intestinal cleanliness, and the precautions before and after colonoscopy. The effective intestinal cleansing rates of the two groups were observed and recorded. Results The average score of intestinal cleaning knowledge assessment of nurses increased from 83.5 before the training to 93.1 after the training; the effective intestinal cleansing rate from July to September of the same year was continuously improved by the training combined rectification, increasing from 73.1% to 92.3%, which was above the target value of 91.2%. Conclusion Intestinal cleaning knowledge training plays a significant role in improvement of the rate of effective intestinal cleaning, which can significantly reduce the rate of missed diagnosis and misdiagnosis.

Abstract: Objective: The study aims to prevent rusting of clinical instruments by using PDCA circulation. Methods: The routine recovery and cleaning rusty instruments management for clinical diagnosis and treatment instruments from October to December in 2018 are chosen as before implementation. And after the PDCA continuous quality improvement measures from January to September in 2019, the implementation of recovery and cleaning rusty instruments management for clinical diagnosis and treatment instruments from October to December in 2019 are chosen as post-experiment. And the rust situation before and after the implementation of clinical diagnosis and treatment instruments will be compared. And the study investigates the satisfaction of clinical departments with the improvement measures of Central sterile supply department before and after the implementation. Results: After implementing PDCA continuous quality improvement measures, the rust rate of clinical diagnosis and treatment instruments was significantly lower than before implementation (P<0.05), and the difference was statistically significant. After implementation, the satisfaction of 58 clinical departments was 100%, significantly higher than that before (100.00% vs 81.03%, P<0.05). Conclusion: PDCA continuous quality improvement measures can effectively prevent the rust of medical devices, improve the quality of instruments of cleaning and sterilization, reduce hospital sense risk and medical cost, improve the satisfaction of clinical departments, and the application value is remarkable.

Abstract: Background: Nursing work environments often expose nurses to toxic behaviors. Today’s nurse leaders face many challenges and concerns in order to empower their staff through creating professional practice environment that require a new type of leadership in which authentic leadership is considered a positive relational-leadership approach, a. Aim: To examine nurses’ perspective of authentic leadership and its relation to structural empowerment and work environment in critical care units at Shebin El-Kom teaching hospital. Subjects and Methods: Non-experimental descriptive correlational research design was employed in the conduction of this study. A simple random sample (244) of nurses having minimum one year of experience composed the study sample. Three standardized questionnaires were utilized which are Authentic Leadership Inventory, Conditions of Work Effectiveness Questionnaire and Practice Environment Scale of the Nursing Work Index. Results: The current study declared that the majority (98.4%) of nurses considered their leaders to be practicing moderate to high authentic leadership. Moreover, (95.1%) of nurses experienced moderate to high levels of structural empowerment. Likewise, about three quarters (75.4%) of nurses perceived high level of work environment. Conclusion and Recommendations: There was a significant positive relation among authentic leadership, structural empowerment and work environment from nurses' perspective. This study can help policy makers, administrators, and nursing leaders to detect areas in need of improvement in nurses’ work environment and handle it, which leads to improvement in quality of nursing care and consequently patient outcomes.

Abstract: Objective: To explore the effects of home care in the elderly patients under the epidemic of COVID-19. Methods: According to the hospital deployment and guidance documents, a series of actions were worked out, including the home care service process and contingency plan, training of the knowledge of novel coronavirus pneumonia (NCP), providing psychological intervention for nurses, providing home care for elderly patients at home and education about home prevention of COVID-19 for the elderly family. Results: From January 19 to March 31, 2020, the home visiting nurses provided home care service for 93 elderly patients at home, representing a 20% increase over the same period last year. The home care service included retention / replacement of gastric tube, retention / replacement of urinary tube, pressure ulcer care, sputum suction care, maintenance of peripherally inserted central catheters and venous blood collection for 127 times. The home visiting nurses were in good physical condition without any bad mood. Conclusion: Under the epidemic of COVID-19, more home-based elderly patients had home care service needs. Home care could not only take advantage under the epidemic situation, but also meet the medical needs of elderly patients at home, and effectively alleviate the burden of epidemic prevention and reduce the risk of cross infection. Home care could play a great role in prevention and control of the COVID-19 epidemic.

Abstract: Objective: To explore the effect of Nursing Quality Improvement on reducing the incidence of hypoglycemia prior to and during colonoscopy in diabetes patients. Methods: The causes of hypoglycemia prior to and during colonoscopy were first analyzed. Then, the corresponding improvement plan and special procedure rules prior to and during colonoscopy were put forward. All the nurses including 23 nurses in the endocrinology department and 2 nurses in the endoscopy room must strictly obey the procedure rules. The Ward of Manager and the Nurse in charge were responsible for supervision. The awareness rate of nurse staff on the knowledge of colonoscopy and behavior improvement of nurse staff before and after the improvement were compared. The incidence of hypoglycemia prior to and during colonoscopy before and after the improvement were calculated. Results: After the Nursing Quality Improvement, the awareness rate of colonoscopy related knowledge in nurse staff was improved from 97.6 to 100%. All the nurses changed their nursing behavior related to bowel preparation from “Occasionally” to “Always”. Besides, the incidence of hypoglycemia prior to and during colonoscopy was reduced from 25% to 8.58% after the improvement. Conclusion: The application of Nursing Quality Improvement prior to and during colonoscopy can reduce the incidence of hypoglycemia and ensure the safety of patients.

Abstract: Background: The birth plan raises women’s knowledge, improves empowerment, and promotes childbirth outcomes. Aim of the study was to implement a birth plan and evaluate its effect on women childbirth outcomes and empowerment. Design: A quasi-experimental comparative design. Sampling: A purposive sample of 194 pregnant women was enrolled and equally allocated into two groups (the intervention group who engaged in a birth plan to receive planned care during childbirth, a control group who received routine hospital care) 97 women each. Setting: The study was conducted at the obstetrics outpatient clinic and in the delivery room of Benha University Hospital. Tools: four tools were used for data collection; A Structured Self-Administration Questionnaire, Birth plan fulfillment sheet, childbirth outcomes sheet, Childbirth related Empowerment Scale. Results: that there were highly significant difference in most items of designed birth plan care during 1st, 2nd and 3rd stage between control and study group (P ≤ 0.001) and there was a significant decrease in mean ± SD of 1st stage duration and the total duration of childbirth stages of the study group comparing to the control group (P ≤ 0.05). Before implementing the birth plan, there was no statistical significance difference between control and study group regarding childbirth-related empowerment scale as (p ˃ 0.05) while after implementation, there were highly statistically significant differences regarding most items of birth-related empowerment scale as (P ≤ 0.001). Conclusion: the birth plan has a higher implementation of designed childbirth care, a positive effect on maternal and fetal outcomes and there was a highly significant increase in the total women`s empowerment scores after implementing birth plan (p<0,001) compared to control group. Recommendation: Raising awareness of healthcare staff of the outpatient clinic, delivery ward to implement birth plan toward increasing childbirth empowerment.

Abstract: Objective: To determine the effect and advantages of no indwelling catheter during perioperative period of microvascular decompression for trigeminal neuralgia under the concept of rapid recovery. Methods: 30 patients underwent microvascular decompression for trigeminal neuralgia at our hospital during February 2017 and December 2019 were collected and divided into experimental group and control group by whether patients adopted urinary catheter or not at the concept of rapid recovery. The data of gender, age, operation time, intraoperative blood loss, urine tract infection, incidence of urinary retention, urinary comfort and average postoperative hospital stay were collected and compared. Results: No significant difference in gender, age, operation time, intraoperative blood loss (P > 0.05) between those two groups. The intraoperative fluid infusion and urine output in the control group were significantly higher than those in the experimental group. There was no significant difference in urine tract infection and urinary retention in these two groups (P > 0.05), comfort level (P < 0.05), and no significant difference in the average postoperative hospital stay (P > 0.05). Conclusions: Microvascular decompression for trigeminal neuralgia without urinary catheter during perioperative period under the concept of rapid recovery is feasible and safe, will not increase risk of urinary retention, improve postoperative comfort and shorten average postoperative hospital stay.

Abstract: Objective: To assess effect of comprehensive nursing intervention on emergency treatment of acute myocardial infarction patients. Methods: 80 patients were invested to join our study who were diagnosed as acute myocardial infarction. They went to the hospital for treatment from March 2017 to July 2019. The participants were randomly assigned to two groups, that included a control group (n = 40) and an intervention group (n = 40). The control group participants received conventional nursing in treatment process. The intervention group participants receive comprehensive nursing intervention. All participants provided the data after their condition stabilized, the data contains first aid time, average hospital stay, the situation of cardiovascular adverse events and patient satisfaction. Result: Base on recording result, first aid time and average hospital stay of intervention group are shorter than that of control group, and those are statistical significance. In cardiovascular adverse events research, intervention group has better performance in cardiovascular adverse, it contains post-infarction angina pectoris [5 (12.5%) vs 10 (25.0%)], heart failure [1 (2.5%) vs 5 (12.5%)], Myocardial infarction recur [1 (2.5%) vs 6 (15.0%)] and cardiac death [0 (0%) vs 1 (2.5%)]. In satisfaction research, intervention group has more very good assessments in the patient report (21 vs 12). Conclusion: the comprehensive nursing intervention has strongly improvement to acute myocardial infarction patients in emergency treatment. Its influence contains not only reducing first aid time and average hospital stay but also reducing the cases of cardiovascular adverse events. Additionally, it also can improve patient satisfaction in the treatment process.

Abstract: Objective: To explore the difference of fall prevention knowledge level of nurses working on special care units and general care units in acute hospitals. Methods: Nurses in special care units and general care units from two acute care hospitals were investigated about fall prevention through a cross-sectional survey. The collected data was analyzed by using Lasso Regression Model. Results: Total of 1442 valid questionnaires were conducted including 1028 questionnaires in general care units and 414 questionnaires in special care units respectively. Through analysis by Lasso Regression Model, seven of 17 fall prevention knowledge items were statistically different in nurses working on general care units and special care units, including the definition of a fall (P=0.0001), fall prevention assessment tool currently used (p=0.0053), assessment of patient's balance and activity (p=0.0020), diseases that easily cause patients to fall (p=0.0021), assessment of patient's foot problems (p=0.0068), assessments of the patient's clothing and footwear suitability (p=0.0011), reporting process of falls (p<0.0001). Conclusion: This study showed that nurses in special care units have higher level theoretical knowledge of fall prevention than nurses in general care units, while having lower level practical knowledge of fall prevention. It suggested that Quality & Patient Safety Department in medical institution should make new plans to prevent patients from falling and ensure patient safety.

Abstract: To investigate the knowledge, attitude, and behavior of nurses toward sexual guidance on patients after THR (total hip replacement) surgery, thus providing data for departments to develop nursing policies for patients after THR. A Web-based survey was completed by 92 nurses from the First Affiliated Hospital of Jinan University. Viability analysis was conducted to evaluate the questionnaire’s reliability. Data were collected and explored by descriptive analysis and analysis of variance to assess the knowledge, attitude, and behavior of nurses with different characteristics toward sexual guidance for patients after THR. The survey showed that the nurses with more working experience got a higher score on knowledge. And orthopedic specialist nurses score higher than non-specialist nurses. The difference was statistically significant (P <0.05). The attitude and behavior scores were very low among the nurses of all groups with different characteristics, they did not reach a total score of 60%, and there was no significant statistical difference between all groups (P> 0.05). In conclusion, the nurses with more working experience or specialize in orthopedic department have a good knowledge of sexual guidance on patients after THR. But nurses with different characteristics have a low score of attitude and behavior scores on of sexual guidance on patients after THR. The clinical department should improve training and emphasize the importance of sexual guidance on patients after THR.
