Abstract: Objective To observe the abnormal expression of IL-1βin hippocampus after bilateral common carotid artery ligation and to evaluate its effects on memory impairment induced by chronic cerebral ischemia. Methods Bilateral common carotid artery ligation (2-vessel occlusion, 2VO) was performed in wild type (Wild Type WT) and interleukin -1 receptor knock-out (IL-1 Resept Knock Out, IL-1R KO) C57 mice (male) to produce chronic cerebral ischemia mouse models. After the operation, their memory was detected with eight-arm radial maze test and the novel object recognition test, and the level of IL-1β and BDNF in hippocampus was detected with Western Blot. Results The average number of working memory errors of the KO-2VO mice was significantly decreased (P<0.05), and the recognition index was significantly increased (P<0.05) compared with those of the WT-2VO mice. Dramatic decrease in IL-1β in hippocampus (P<0.05) and substantial increase in BDNF (P<0.05) were observed in the KO-2VO mice. In the behavioral experiment and Western Blot, there was no significant difference between the KO-sham mice and the WT-sham controls. Conclusion Increase in the level of the IL-1β in hippocampus after bilateral common carotid artery ligation can lead to decrease in the level of the BDNF, which may cause memory impairment.

Abstract: Behcet’s disease (BD) is a multisystemic inflammatory disorder of unknown etiology. There is few data about BD in Saudi Arabia. This is a retrospective chart review study of 28 patients of BD with thrombosis on long term anticoagulation. A total of 28 BD patients, their mean age was 43.8 (25-60) years. The mean age of first thrombosis was 29.2 (14-42) years. Provoked thrombosis was reported in 17.86% of the patients. The commonest site of thrombosis was superficial femoral vein 35.71%, followed by popliteal vein 32.14%, pulmonary embolism (PE) 32.14%. Anti B2 glycoprotein 1 antibodies was identified in 31.82% of patients, Anti cardiolipin Antibodies in 22.73%, Leiden factor V in 18.18%. Activity of the disease during thrombosis was recognized as oral ulcer 44.44%, genital ulcer 33.33%, Human leukocyte antigen-B (HLAB) was 32.14%. Thrombosis while patient on anticoagulation was reported in 32.14% of patients. Bleeding was reported in 14.29% of patients, common site of bleeding was gastrointestinal bleeding. Survival rate was 78.57% and 2 patients were died (7.14%) due to malignancy. Conclusion: Thrombosis in BD patients is male predominance, more in venous, has low mortality, frequency of thrombosis is more in the presence of thrombophilia and active BD. The prevalence of thrombophilia among BD patients is high. The presence of thrombophilia still affect the physician decision regarding the initiation and duration of anticoagulation in BD. The incidence of bleeding with long term anticoagulation is low. Further research is needed to find out the prevalence of cancer among BD in Saudis.

Abstract: The objective of this paper is to describe the clinical and therapeutic characteristics of thrombosis associated with cancers in patients in charge of the internal medicine-oncology department of the Ibn Sina Military Hospital in Marrakech. This is a retrospective, descriptive and analytical study over a two-year period from January 2013 to December 2014. Among 158 patients with cancer, we identified 25 with thrombosis confirmed by imaging. Characteristics of the population of origin, incidence, risk factors for venous thromboembolic disease (VTE), diagnostic means, distribution of thrombosis, location and histological type of cancer and therapeutic strategy were analyzed. The mean age of our patients was 55.89 years, with female predominance (56% vs 44%). The incidence of venous thrombosis in our series is 15.8% (n = 25) of which 76% (n = 19) are isolated, 12% (n = 3) are on central venous catheter (CVC) and 12% (n = 3) are associated with pulmonary embolism. The lower limbs were the most affected (80.3%), with predominance on the left side. Bilateral involvement was found in 7% of cases. Thrombosis was indicative of cancer in 16% of patients (n = 4). While it complicated the development in 84% of the cases (n = 21). Risk factors for VTE were dominated by females (56%), metastases (64%), chemotherapy (80%), and the presence of a CVC (80%). Solid tumors were the most frequent 72% (n = 18) with predominantly digestive cancers (28%), pulmonary (16%), bladder (16%) and breast (12%). The most predominant histological type was adenocarcinoma in 44% (n = 11). The seven cases of malignant hemopathies are: four non-Hodgkin's lymphomas, two multiple myelomas and one case of chronic myeloid leukemia. The initial curative treatment as a relay used low molecular weight heparin in 92% of patients (n = 23). Unfractionated heparin was used in two patients with severe renal failure, the only ones receiving vitamin K antagonist maintenance therapy. Short-term outcomes were favorable in 92% (n = 23), two deaths Have been attributed to severe pulmonary embolism. Significant progress has been made in the context of cancer-related VTE. Nevertheless, there remains a considerable field of unresolved clinical issues. Numerous studies are underway and may lead to advances not only in the treatment of VTE associated with cancer but also in the treatment of the cancerous disease itself.

Effects of Postoperative Methycobal Administration on Digital Replantation Based on Stellate Ganglion Stimulation

Abstract: To study the impact of stellate ganglion block (SGB) on digital replantation and analyze its mechanism. Digital arterial diameter was enlarged after SGB, and the difference in the survival rate of severed finger after replantation was not statistically significant between the two groups (P>0.05). Through the treatment with Methycobal, the recovery of 2-point discrimination (2-pd) of the experimental groups (A, B) was significantly better than that of the negative control group, between which the difference was statistically significant (P<0.01 between Groups A and C, P<0.05 between Groups B and C), indicating that the methyl vitamin B 12 can significantly promote the regeneration of peripheral nerves. Methycobal is of great significance to the postoperative treatment of digital replantation, which is worthy of being widely applied in clinic practice.
