Abstract: We have examined 40 patients with bronchial asthma (BA) based at the municipal non-profit enterprise "Ivano-Frankivsk Regional Phthisiopulmonology Center of the Ivano-Frankivsk Regional Council" (Center for Pulmonary Diseases). The patients were randomized into two groups: Group I (17) involved patients with BA, Group II included (23) patients suffering from BA with obesity. The goal of the research work was to analyze the manifestations of clinical symptomatology, manageability of BA, state of pulmonary function, level of leptin in relation to mono- or polymorbidity. The research findings show the decrease in asthma manageability level in patients with combined pathology, particularly BA + obesity + gastroesophageal reflux disease (GERD), significant negative correlation (r=-0.95, p<0.001) between FEV1 and ACQ-5 data in patients suffering from BA with obesity and GERD. The frequency of nocturnal asthma attacks, heartburn, and sleep disorders tended to increase in patients with comorbid conditions. Features of changes in the concentration of leptin depending on the detected pathology is characterized by the following data. The level of leptin in patients with BA with and without obesity is 4.82 times higher as compared to patients with combined pathology (45.28 ± 10.42 ng/ml), and is significantly different (p<0.05) from the indices observed in patients with normal body weight (9.40 ± 1.52 ng/ml).

Abstract: Patient education about medications being used highly improves the quality of proper use and compliance, to which the WHO has created guidelines of good prescribing. The main objective of this observational study is to assess the effectiveness of physician counseling and their compliance to the WHO guidelines to good prescribing when given the chance to prescribing new medications to patients coming for a follow up in various clinics in the Bahrain Defense Force Hospital in the form of a survey over a period of two weeks. 401 patient interviews were randomly chosen and included in the data analysis. 33 physicians were assigned in the survey, from which there were 7 cardiologists, 5 general practitioners, 3 diabetologists, 3 dermatologists, 2 endocrinologists, 5 pediatricians, 3 vascular surgeons, and 5 internists. The survey was developed based on the standards mentioned in the WHO guidelines. The main outcome was to observe and assess how efficient are physicians from various clinics in successfully prescribing new medications to patients through ten WHO standards. Data revealed that in general and for the most part the quality of instructions and information given to patients while prescribing new medications was relatively unsatisfactory in most clinics, although it was found that prescribing patterns differs from one physician to the other.

Abstract: Primary myocardial involvement is common in systemic sclerosis. There is growing evidence strongly suggesting that this involvement is linked to repeated focal ischemic lesions causing irreversible myocardial fibrosis. It can affect all heart structures. We report the case of a 65 years old male patient, without any personal medical history, admitted for inaugural right heart decompensation. The diagnosis of restrictive cardiomyopathy is based mainly on the following criteria: on echocardiography, hypertrohy of the right ventricle with alteration of the longitudinal systolic function, dilation of the right atrium, with good overall contractility. On right cardiac catheterization: elevation and equalization of the telesystolic pressures of the right atrium, right ventricle, and pulmonary artery with appearance of dip plateau on the right ventricle curve. Scleroderma was evoked considering following statements: the presence of Raynaud's phenomenon, unknown by the patient (presence of dermal sclerosis with pericapillary oedema and capillary dystrophy on capillaroscopy), cutaneous involvement (presence of perivascular and perineural mononuclear infiltrates, with collagen fibers increased in number and thickness), pulmonary involvement (diffuse bilateral interstitial lung disease) and renal involvement (moderately impaired function and positive proteinuria). The concept of cardiac dysfunction in scleroderma and other rheumatologic conditions has received new interest with the advent of newer non-invasive imaging techniques. Therfore, it would be necessary to search the cardiac involvement especially subclinical one in this type of system disease, as well as to confirm the systemic origin in front of certain forms of cardiopathies in particular restrictive cardiomyopathy.

Abstract: The investigation is devoted to the problem of improvement of diagnostics and treatment of the somatopsychic phenotype (SPPh) at patients with bronchial asthma (BA). On the basis of complex clinical, instrumental, psychological inspection, investigation of the parameters of life quality by MOS SF-36 questionnaire of 207 BA-patients certain prevalence of the combination of concomitant psycho-emotional disorders in 39.1% of BA patients, such as high levels of anxiety, neurotism, clinically significant depressive symptoms and somatopsychological accentuations (pedantry, emotionality, anxiety, excitability, cyclothymia and dysthymia). It is established, that BA patients have the somatopsychic phenotype of the course of the disease. We described the features of clinical course of BA, lung functional parameters and indexes of quality of life at patients with the pathologically changed somatopsychological descriptions. It is associated with an increase in the clinical severity of the disease, a decrease in the lung function parameters, low quality of life indexes and absence of asthma control. Assignment of psychotropic drugs (fabomotisol dihydrochloride (afobazole) or piracetam/thiotriazoline) in the complex treatment leads to stabilization of the clinical and psychological state of patients, increasing the level of control of asthma. The clinical and economic effectiveness of using of the combined modes of therapy with including of preparations of psychotropic action on a base treatment was proved.

Abstract: To compare different methods of estimating glomerular filtration rate (GFR) calculated by creatinine and cystatin C in patients with type 1 diabetes with normal and moderately decreased renal filtration function. The study involved 57 patients with type 1 diabetes, 37 men and 20 women, aged from 21 to 57 years (median 30 years), with disease duration after diagnosis from 33 to 2 years (median 8 years). The control group comprised 15 non-diabetic people aged 19 to 42 years (median – 28 years), with normal levels of albuminuria and blood creatinine. The majority of patients with type 1 diabetes were found to have decompensated diabetes mellitus. The mean value of glycated hemoglobin in patients was 8%. The renal glomerular function was estimated by level of GFR and triple testing of urine albumin excretion. Renal tubular function in type 1 diabetes was tested by examining serum cystatin C. GFR estimating equations, which did not include serum of cystatin C produced higher GFR. As a result, the majority of patients had normal or elevated GFR showing hyperfiltration. Using cystatin-C-based equations resulted in a several-fold reduction in the number of patients with hyperfiltration and increased number of cases with GFR below 90 ml/min/m2. The assessment of the relationship between SDMA and GFR showed negative correlation with both cystatin-C-based and creatinine-based equations. These findings revealed that GFR estimating equations based on both creatinine and cystatin C produce more accurate results compared with the reference estimating equations.

Abstract: Objective: We attempt to explore the effect of mindfulness-based stress reduction on unhealthy emotion and sleep quality of hematopoietic stem cell transplantation recipients. Methods: we enrolled 92 patients who underwent hematopoietic stem cell transplantation recipients in the Hematology Department of our hospital from February 2017 to August 2019 and averagely divided them into control group and observation group according to random number table. Patients in the control group received routine nursing care for hematopoietic stem cell transplantation recipients while patients in the observation group were given mindfulness-based stress reduction for mental intervention based on the routine nursing care. We adopted Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), and Pittsburgh Sleep Quality Index (PSQI) to assess the patients’ emotional state and sleep quality before and after intervention. Results: Before intervention, there was no significant difference in the score on SAS, SDS, and PSQI between two groups (P>0.05). After intervention, the scores on SAS, SDS, and PSQI of both groups declined obviously and that of the observation group declined much more than that of the control group. There was a significant difference in the score on SAS, SDS, and PSQI between two groups after intervention (P<0.05). Conclusions: Mindfulness-based stress reduction can relieve unhealthy emotions such as anxiety and depression of hematopoietic stem cell transplantation recipients as well as their mental stress so as to improve their sleep quality.
