Abstract: Research background: In the world, youth and beauty are what everyone likes to pursue. In the medical discipline, the medical cosmetology discipline has gradually formed and become powerful. Our research is to learn the application of L-polylactic acid injection therapy in anti-aging therapy. We used L-polylactic acid injection therapy to compare the beauty effect on the pictures before and after the anti-aging treatment of patients. It provides research data and basis for the anti-aging effect of L-polylactic acid injection therapy in subsequent cosmetic medicine. Research Purpose: to expand the field of knowledge and skills for medical cosmetology, and lay a medical research data and theoretical foundation for the field of anti-aging treatment. Research method: This study is to explore a patient who requires left-polylactic acid beauty, anti-aging and filling treatment. Study the changes of their faces before and after surgery; after treatment. Draw conclusions, study and discuss. Research Results: a cosmetic patient with L-polylactic acid had a significant beauty effect after anti-aging and filling treatment, and no one had bleeding, necrosis, infection, allergy and other complications. Research Conclusion: L-polylactic acid has only an immediate filling effect, and cannot have the therapeutic effect of hyaluronic acid, that is, the long-term (3-12 months) filling effect. It mainly relies on L-polylactic acid to stimulate tissue after entering the human body to produce regeneration, so as to achieve the filling beauty effect. L-polylactic acid injection for anti-aging and filling has a certain limit in a single stimulation. It requires 2 to 5 times of multiple stimulation to achieve the patient's satisfactory effect. The interval between each stimulation is about 3-6 months. The effect of the patient varies according to the patient's own situation (under the age of 40) and the better the regeneration ability. The older you are, the worse the effect. The fatter the person, the better the effect and the better the regeneration ability. The thinner the physique, the worse the effect. The effect of patients varies according to the doctor's dosage. When using a double dose, the effect is more obvious on the same individual than the single dose.

Abstract: Introduction: Rectal prolapse is defined as the full or partial thickness concentric protrusion of the rectum or rectosigmoid via the anus. This is a growing clinical concern that is usually found in elderly patients. The aim of the study was to report the results of surgery in two patients operated on for strangulated rectal prolapse using the ALTEMEIER technique and to review the literature. Observation 1: 78-year-old grandmother is admitted for a painful and irreducible rectal protrusion evolving for 72 hours. Given the failure of self-medication with poultices, she consults our service. In this history, she had stubborn constipation. She was a lucid patient. We saw a voluminous perineal mass, irreducible necrotic in places, with a polyp at its top. The examination of other devices as well as the biological assessment did not present any particularity. A rectosigmoidectomy with colo-anal anastomosis was straightforward. The patient was without complications six months later. Anatomo-pathological examination of the surgical specimen was not available. Observation 2: A 51-year-old merchant, admitted for a painful, irreducible rectal protrusion evolving for 48 hours. He consults after several unsuccessful attempts at self-reduction. He had stubborn constipation and bilateral inguinal herniorrhaphy as a history. Examination showed a large perineal mass with ischemic necrosis. Elsewhere it was unremarkable. Paraclinical assessment revealed hyperleukocytosis and accelerated ESR. The diagnosis of total strangulated rectal prolapse was made. Under spinal anesthesia, a rectosigmoidectomy with colo-anal anastomosis was performed immediately. The patient seen again six months later had no complications. Conclusion: strangulated rectal prolapse is a surgical emergency. Its PEC takes into account the patient's condition and the surgeon's experience. Perineal resection is the preferred surgical option in emergencies although its recurrence rate is higher compared to its cure through the abdominal route.

Abstract: Introduction: In most African public hospitals, emergencies remain a frequent mode of admission. The aim of this study was to describe the epidemiological profile and management of surgical emergencies at ANAIM hospital in Kamsar (Guinea). Patients and methods: this was a prospective descriptive study lasting six months (October 2021 to April 2022) in the general surgery department at ANAIM hospital in Kamsar, covering all patients admitted for emergency surgery in the surgery department during the study period. Results: During the six-month study period, surgical emergencies accounted for 16.92% (n=203) of all admissions (N=1200). The mean age of patients was 29.87%, with extremes of 1 and 85 years. The sex ratio was 2.38, with males predominating (70%). Housewives were the most affected (27.10%). Acute appendicitis (24.1%), limb fractures (16.3%), acute bowel obstruction (13.3%) and acute peritonitis (11.8%) were the more frequent emergencies. The morbidity consisted mainly of surgical site infection (14.3%) and incisional hernia (3.4%). The overall mortality was 8.9%. Conclusion: Surgical emergencies, dominated by infectious diseases and fractures, are a major concern at ANAIM hospital in Kamsar. Our data showed that show clearly that late management because of lack of emergency kits and experienced surgeons increases the risk of postoperative complications and death. Thus, supplying the hospital with emergency kits and resuscitation means could considerably improve the quality of care for these patients.

Abstract: Introduction: Malignant soft tissue tumours (MSCTs) are mainly represented by sarcomas, and their incidence varies. The aim of this study was to describe the surgical treatment of MPMT in the surgical oncology unit of the Donka National Hospital. Materials and Methods: This was a retrospective study carried out over a period of 14 years (2007 to 2021), covering patients with histologically confirmed malignant soft tissue tumours who had received specific treatment. Results: We compiled 83 records of patients with histologically confirmed soft tissue tumours, 12 (14.4%) of whom were treated surgically and 15 (18.1%) medically with chemotherapy. The mean age was 39.4 years. The 40-49 age group accounted for 6 (22.2%) cases. Women predominated in 14 (51.8%) cases. The average consultation time was 26.2 months. The thigh and the knee were the sites represented in 7 (26.0%) and 6 (22.2%) cases respectively. Fibrosarcoma was the most common histological type in 12 (44.4%) cases, followed by rhabdomyosarcoma in 9 (33.3%) cases. Clinical stages III and VI accounted for 13 (48.1%) and 8 (29.6%) cases respectively. Seven (58.3%) cases underwent wide excision and 5 (41.7%) cases underwent amputation. Node dissection was inguinal in 5 (100.0%) cases. Resection was R0 in 8 (66.6%) cases. Post-operative follow-up was good in 11 (91.6%) cases. One case of lymphoedema of the limb and chronic pain was recorded. Conclusion: The management of malignant soft tissue tumours is fraught with difficulties in our context, as the diagnosis is usually made at a late stage. Early, planned and correctly performed initial surgical treatment has a major impact on prognosis.

Abstract: Objective: This study aims to identify factors associated with death in seriously ill patients with SARS CoV-2 pneumonia who required a tracheostomy and the time in which the procedure was performed. Patients and methods: The study was observational, analytical, prospective, cross-sectional, and case-control in nature. The decision to perform a tracheostomy was based on the clinical conditions and the approval of the Institutional COVID Committee. To compare means of continuous numerical variables, paired and unpaired Student's t test was used depending on the variables analyzed; to establish correlation between continuous numerical variables, the Pearson correlation coefficient was calculated; to analyze the probability of death with respect to the variables studied, the odds ratio was calculated with a 95% compatibility interval. Results: a total of 47 severely ill patients with COVID-19, who were admitted to the Intensive Care Unit, and required a tracheostomy, 17 women and 30 men, underwent an open tracheostomy. The population was predominantly male, and blood type A+ was more common. Most of them had at least one of the following comorbidities: Diabetes, hypertension, obesity or smoke. Only 10 patients survived until the end of following. All tracheostomies were performed in the first days of the illness, even in the first seven days, except for one patient. All tracheostomies were performed openly, almost all within a conventional operating room and performed by General Surgeons; We have not had any early or late complications, although the risk of tracheal stenosis remains latent; none of the surgeons presented symptoms of the disease, although detection test were never applied to us. Conclusion: In this group of patients, diabetes, systemic arterial hypertension, obesity, blood type "A" and age over 65 years were associated with higher mortality, while no improvement was shown when tracheostomy was performed early, even the first 7 days.

Abstract: We present a new method for reconstruction of chronic neglected tears of the deltoid ligament of the ankle joint, using half of the posterior tibialis tendon. This has been applied to 5 patients with an average follow-up of 6 years with very encouraging results. Five patients with a mean age of 28.2 years (range 23-50 years) visited us with chronic medial collateral ligament rupture. All were treated surgically where the deltoid ligament was reconstructed with half of the posterior tibialis tendon. Median time since injury was nine months (range 3-17 months), cause of rupture was sports injury and the preoperative AOFAS score 50,4 (range 48–64). All patients were followed up postoperatively for a mean of 3 years (range 1-5 years) and had a stable ankle with mean plantar flexion of 50° and dorsal flexion 20º. No complications occurred. The postoperative AOFAS score was 91,8 (range 85–97). Deltoid ligament reconstruction using half of the posterior tibialis tendon is an effective method of treatment for chronic neglected tears of the deltoid ligament. This treatment option has very good results, it closely mimics the anatomy of the deltoid ligament, the graft is strong, it is economical without the use of allografts and artificial grafts.
