Abstract: Congenital true aneurisms of peripheral arteries especially on the hand are rare and idiopathic. The present case is focused on the multiple aneurysms of a young boy on both radial and ulnar arteries since birth. The clinical indications include painful ulcers on the right middle finger, discoloration and swelling of dorsum of ulnar side of right hand. In addition, patchy necrosis of fifth finger with increased intensity of pain was observed. Here we successfully treated both aneurysms with partial amputation of the digit and the growth of the skin on both sides of the palm was restored by groin flap.

Abstract: The aim of this study is to reveal the etiologic link that exists between pancreatic cancer and chronic pancreatitis through our unicentric experience. The clinical and cyto-pathological results of 300 patients were studied prospectively from January to December 2014 in the pancreatic center of Wuhan union hospital in china. Of the 300 pancreatic lesions patients, 196 were male and 104 female with a sex ratio of 1.8:1 (p=0.001). The factors assessed for the determination of etiologic link were the age, gender, the diameter of the lesion, the antecedents of disease (such as ulcer,cholecystectomy, smoking ,alcohol abuse ),the pancreatic lesion site, the diabetic link, the clinical presentations, the level of transaminases (GGT, ALP), peripheral blood analysis and cytopathological results of both PC and CP groups . According to the clinical data along with the respective results of the examinations, causal factors that link CP and PC are illustrated and it opens the pathway for further detailed studies as an investment for the future.

Abstract: Background: The forehead flap is a useful technique to reconstruct deep and large nasal defects. Its disadvantages includes the fact that it is at least a two-stage procedure .The aim of this work is to describe our experience with the use of One stage forehead flap reconstruction to cover nasal defects. Patient: Fifteen patients with extensive nasal defects due to various causes. Full-thickness skin was lost in all cases, n=7 cases after excision due to rodent ulcer, n=5 cases after squamous cell carcinoma and n=3 cases after nasal trauma. Ulcer sizes and sites were demonstrated in table 1. All were repaired with a single stage forehead flap. The functional and aesthetic results were assessed. Results: High aesthetic and functional goals were achieved in all patients. There were no cases of significant flap necrosis. However, one patient developed mild superficial partial-thickness necrosis. Conclusions: The singe stage forehead flap represents one of the best methods for repair of extensive nasal defects, which may be a substitute for the traditional method (two stages) regard to time repair and cosmetic results.

Abstract: We studied the impact on prognosis by negative LN (NLN) harvest count on rectal cancer patients, using immunohistochemistry to exclude micrometastasis in nodes negative by hematoxylin and eosin (H & E) stain. A prospective study including 114 patients in Mansoura university hospitals between 2005-2008 aged 18 years or older who were diagnosed as having localized invasive adenocarcinoma rectum with stage I, II & III was conducted. Close follow up done in both surgery department and department of clinical oncology and nuclear medicine for 5 years. We examined patient survival in relation to the negative lymph node count. Our results revealed that the number of negative lymph nodes could affect significantly survival curves where the disease free survival and overall survival were significantly better among the group of patients with more than ten negative LNs (P=0.021 & 0.012 respectively). In conclusion, the negative lymph node count is associated with improved survival of rectal cancer patients.

Influence of Preoperative Disease Course on the Quality of Life of Adolescent Idiopathic Scoliosis Patients

Abstract: To study the influence of preoperative disease course on the operation and postoperative quality of life of adolescent idiopathic scoliosis (AIS) patients. Patients who were treated with simple posterior correction and pedicle-screw internal fixation were divided into two groups according to their preoperative disease courses: a short course group with preoperative course < 2 years (S group), and a long course group with preoperative course ≥ 2 years (L group). The gender, Lenke type, and major curve cob angle were matchable between the two groups, and 112 cases were included in each group. Various radiographic measurements and indices like fusion level, intraoperative blood loss and blood transfusion scores of SRS-22 were compared between the two groups during preoperative, postoperative and follow-up periods. The preoperative side scoliosis angle Cobb in S group was less than that in L group (P=0.040). The coronal plane and sagittal plane radiographic parameters were similar after operation, and there were no statistical differences between the volume of intraoperative blood loss and blood transfusion. The preoperative major scoliosis Cobb angles were similar between the two groups, but the flexibility of the major scoliosis in S group was larger than that in L group (P=0.039). The number of fused vertebrae in L group was higher than that in S group (P=0.024). The function/activity, pain, self-image/appearance, and mental health in the SRS-22 scales of the two groups had no statistical differences during follow-up, and L group had a lower satisfaction rate of treatment compared with S group (P = 0.037). The scoliosis flexibility decreased with increasing disease course. Disease course might be a risk factor for side scoliosis progression and it affected the quality of life of AIS patients after operation.
