Abstract: The aim of this article is to describe the pathophysiology, diagnosis, classification, and management of BDI based on the relevant available literature, in particular the recent recommendations from the European Society of Gastrointestinal Endoscopy (ESGE). It is a known fact that bile duct injuries (BDI) are associated with a high morbidity and mortality, posing impaired quality of life along with substantial financial burdens to patients and the society in general. Depending on the type of duct injury, successful management is based upon the time of recognition of injury, patient condition, presence of complications and availability of professional expertise (radiologists, endoscopists and hepato-biliary surgeons). Appropriate management may include endoscopic, per-cutaneous and surgical interventions with imaging playing a significant role in initial diagnosis, assessment and treatment of such injuries.

Abstract: Introduction: Solid primary tumors of mesenteric origin are quite rare among the intraabdominal soft tissue tumors. The most frequent tumors are Gastrointestinal Stromal Tumors (GIST) and smooth muscle tumors. GISTs as the largest group of these tumors are usually seen in the wall of gastrointestinal tract and 25% of them are malignant. They usually occur in adults and there is no sexual predominance. We present a huge intra-abdominal myoma clinically mimicking GIST. Case Report: A 49-years old male was referred to our deparment because of a growing mass and pain in the right lower quadrant. Physical examination revealed a huge mass with 10 cm in diameter in the right lower quadrant. Computed tomography (CT) revealed a mass with 17 cm in diameter which originated from caecum and extented from umblicus to urinary bladder and right colon. The mass was resected en-bloc with right hemicolectomy. The final diagnosis is ‘’Fibromyoma of the mesentery’’. Discussion: They usually arise de novo and are seen in adults. Myofibroma can occur as a solitary form or it can be seen as multiple lesions like generalized form which is described as myofibromatozis. Myofibromas are characteristically circumscribed masses with a nodular or multinodular growth pattern and they usually show a biphasic growth pattern . The behaviour of mesenteric smooth muscle tumours is unpredictable. Because they arise large diameters and they can cause different clinical symptoms like obstruction and abdominal pain. The treatment of mesenteric fibromyoma is complete resection. Large intraabdominal solid masses pose a diagnostic challenge either grossly or microscopically. Histopathologically, huge benign fibromyomas should be kept in mind of differantial diagnosis of GİSTs.

Abstract: As their face ages, a significant number of individuals require elevation of the eyebrows to improve brow-ptosis and pseudo-ptosis of the upper eyelids. This may be for functional disability, i.e. when there is a reduction in visual fields, or for purely cosmetic reasons. Eyebrow elevation can be achieved by non-surgical or by direct surgical methods. There are advantages and disadvantages to the non-surgical use of Botulinum Toxin or fillers, and minimally invasive techniques such as endoscopic methods or Endotine® fixation. Bicoronal brow lift procedures appear to be reducing in popularity compared to the other methods. Suture suspension of the brow is not new but recently Minitac®, a kit system that allows intra-osseous fixation using two non-absorbable paired needle sutures, has been introduced. Whilst this system was primarily developed for ligament to bone approximation, we demonstrated in this mini-series that it can be adapted to provide a simple and effective means for brow lifting, whilst controlling the vectors of elevation. Due care and the understanding of anatomy are essential.

Abstract: Coronary artery ectasia (CAE) is detected in approximately 1-5% of all coronary angiography procedures. Coronary artery dilatation is only rarely diagnosed or cannot be recognized generally. There is no consensus regarding the definition of this rare anomaly. A 75-year old female who is a hypertansive and hyperlipidemia with a history of chronic stable angina despite full medical therapy. She was admitted on intensive care unit this time because of a typical chest pain that did not alleviate with sublingual nitroglycerin. Coronary angiography revealed mainly of the diffuse ectasia of three coronary artery and rheumatic heart valve disease. She underwent aortic and mitral replacement and a three-vessel bypass. Her postoperative course was well and she was discharged on the postoperative day 7 th. The patient is doing well on follow-up with no recurrence of symptoms.

Abstract: Endometriosis causing intestinal obstruction presents a difficult challenge in making a timely and accurate clinical diagnosis because of the non-specific nature of its presentation which can mimic other causes of intestinal obstruction. We report a case of large bowel obstruction and rectal bleeding secondary to multifocal intestinal endometriosis which was thought to be colorectal cancer. Patient had partial colectomy done and histology showed endometriosis. Intestinal endometriosis should be considered as a differential diagnosis in a young patient with recurrent lower abdominal pains and distension in conjunction with signs of obstruction.

Abstract: Aims: We are trying to understand the role of HER 2 neu in tumor progression. Study design: Prospective study. Place and duration: Conducted at Bani Swif and Kasr AL Aini university hospitals between May 2013 and May 2014. Methodology: 32 patients were enrolled in the study. The speceimens were obtained surgically .Expression of HER-2-neu receptors done aided with Dako Code No. A0485. Results: In the study there were 15 cases with pure invasive carcinoma and 17 cases with in situ component , there were more positive cases (59%) with HER-2-neu in the invasive associated with in situ group than those in the pure invasive group (20%), this was statistically significant (P<0.016). Conclusion: In conclusion, it may be assumed that during cells of the in situ component are acquiring invasiveness, they partly loose Her-2-neu over expression The high percent of positive HER-2-neu in cases associated with in situ component point an arrow with question mark towards possible changes in the tumor cell biology or the actual cell line of the in situ and the invasive components.
