A Case Report of Functional Acupuncture in the Treatment of Senile Left Persistent Facial Paralysis

Abstract: Introduction: Idiopathic facial palsy, also known as Bell's palsy (BP) or facial neuritis, is an acute onset of peripheral facial neuropathy. The onset of this disease is rapid, and there is a history of cold, mostly on one side of the face. Objective: to introduce and analyze the method and curative effect of functional acupuncture on one case of senile left persistent facial paralysis. Methods: focus on the functional parts of the affected area that need to be improved, select the acupuncture areas with improved trigger function, and select the stimulation dose of electroacupuncture. In other words, functional acupuncture is the main acupuncture method in the treatment process, that is, to take a functional position before acupuncture on the paralyzed side of the cheek, that is, to lift the patient's left corner of the mouth outward to above the functional position with a surgical small retractor (the largest lifting mouth angle is tilted to the left side), to determine the left cheek muscle functional area (needle entry area), and to make corresponding adjustment on the left cheek Acupoints selected in functional areas: Xiaguan, zygomatic Luo, buccal Che, Yingxiang, Renzhong, Taiyang, sizhukong, Chengzhi, yangbai. The needle of 0.35 mm × 40 mm was selected, and 0.3-1 inch flat needling was used with electroacupuncture. The waveform was density wave. Five times a week, 10 times for a course of treatment. Results: after two courses of functional acupuncture treatment, the patient's left eyelid was basically closed, and the air leakage during the left cheek bulging was significantly improved. After eating, the food left in the left cheek disappeared, the left nasolabial groove appeared, the left frontal lines appeared, and the symptoms of the left mouth angle slanting to the right were significantly improved (see the figure below). Conclusion: the essence of functional acupuncture improvement is to re form a strong neuromuscular effect in muscles, destroy the paralysis and weakness of target muscles, and reconstruct the normal neuromuscular coupling mode. The aim of this balance mechanism is to destroy the reflex pattern of abnormal muscle paralysis and muscle weakness and reconstruct an orderly and normal neuromuscular reflex.

Abstract: Objective: Hand, foot and mouth disease (HFMD) is an acute infectious disease caused by enterovirus 71 (EV71), Coxsackie virus A16 (CA16) and other enteroviruses. For the treatment of HFMD, there are no recognized specific treatment drugs. In recent years, with the continuous development of traditional Chinese medicine, the clinical use of traditional Chinese medicine in the treatment of HFMD reports is increasing. At present, traditional Chinese medicine is mainly used to disperse wind and heat, clear heat and detoxify, clearing damp and dispel evil, but most of them do not know the exact mechanism of treatment. In this paper, the network pharmacology method was adopted to analyze the main active components and action targets of Sang-Ju-Yin (SJY) and to construct corresponding pathways, and to explore the mechanism of action of SJY in the treatment of HFMD. Methods: The active components of SJY were collected and potential targets were searched by the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). GeneCards platform was used to find disease targets, and a protein interaction network (PPI) was constructed using the STRING platform. Cytoscape 3.6.0 software was used to screen out the key targets. The enrichment analysis of KEGG pathway and gene function analysis (go) was carried out by R language bioconducor package. Results: There were 151 main active components such as quercetin, luteolin and wogonin, and 15 intersection targets were obtained after 78 targets and 100 hand foot mouth disease targets intersected. Key pathways such as TNF signaling pathway, measures and influenza A were obtained by KEGG analysis. Conclusions: The main active components of SJY are quercetin, luteolin, wogonin, kaempferol, aloe emodin, Licochalcone A. It mainly regulates AKT1, Bax, IKBKB, IL-6, STAT3 and other targets, regulates TNF, influenza A and other signaling pathways to inhibit inflammatory response and regulate immune function, so as to achieve the purpose of treating hand foot mouth disease.

Abstract: Objective: During the preparation of Zishen Yutai Pills, 5-Hydroxymethylfurfural may be used. To establish a HPLC method for determination of 5-HMF in Zishen Yutai pill and to provide the potential quality control standard of it. Methods: RP-HPLC method was applied with the chromatographic condition as follows: the chromatographic column was Phenomenex Gemini C18 column (250×4.6mm, 5μm), methanol-water (10:90) as the mobile phrase, the flow rate was 0.8mL/min, the detection wavelength was 284nm, the column temperature was 30°C, and the injection volume was 10μL. Results: The calibration curve was linear in the range of 2.16~43.27μg. mL-1 for 5-HMF (r2=0.9993). The RSD of the method repeatability was 0.97% (n=6) and the average recovery was 99.4% (RSD=0.58%). What’s more, the specificity, precision and stability results were also good. Conclusion: The method for HPLC is not only sensitive and accurate. It is also easy to operate which can be used as the determination method for 5-HMF and has a guiding role for the quality control of the Zishen Yutai pill.

Abstract: Objective: To assess treatment effect of auricular acupressure combined with evodia rutaecarpa powder paste Yongquan point in patient with insomnia of disharmony between heart and kidney. Methods: From January 2019 to December 2020, we collected valid data from 106 patient with insomnia of disharmony between heart and kidney. Patients were randomly divided into control group and intervention group according to the order of hospitalization. The control group consisted of 27 males and 26 females, and their average age was 60.23±5.62 years old. The participants of control group received common treatment. On the other hand, intervention group participants received treatment of auricular acupressure combined with Evodia rutaecarpa powder paste Yongquan point, they consisted of 29 males and 24 females, and their average age was 59.35±9.58 years old. Result: After treatment, intervention group had lower points in assessment of TCM treatment compare with control group. Before treatment, sleep quality assessments of two groups were no significantly different (p > 0.05). After treatment, intervention group participants had better outcome in sleep quality than that of control group participants (p < 0.05). Also, in total treatment efficiency, higher percentage of total treatment efficiency in intervention group than in control group [49 (92.45%) vs 42 (79.25%)], and they were significantly different (p < 0.05). Conclusion: treatment effect of auricular acupressure combined with evodia rutaecarpa powder paste Yongquan point is better than common treatment for patient with insomnia of disharmony between heart and kidney. Compared with common treatment, this treatment not only had better effect for sleep quality of patient but also had better treatment efficiency.
