A Randomized, Double-Blind Clinical Trial for Effect of Wisconsin Ginseng (Panax quinquefolius) in Improvement of Rheumatoid Arthritis Associated Fatigue

Abstract: Purpose: To observe and evaluate the clinical efficacy of the Wisconsin ginseng (Panax quinquefolius) herbal botanical adjunctive treatment to improve Rheumatoid Arthritis (RA) fatigue. A secondary aim was to observe RA related biomarkers and evaluate the safety of the treatment. Methods: Eligible adult patients with RA were enrolled between September 2019 and January 2020 at a multisite in a double-blind randomized trial with each intervention lasting 8 weeks with prescription medications, diet, and lifestyle kept constant. Interventions consisted of Wisconsin ginseng in doses of 3,000, or 6,000 mg/day or placebo given in twice daily dosing over 8 weeks. Primary endpoint outcome measures were changes from baseline at 4 and 8 weeks utilizing the Visual analogue scale to evaluate fatigue severity (VAS-F) and Disease activity score 28-joint count erythrocyte sedimentation rate & C-reactive protein (DAS28 ESR & CRP), secondary objectives included The health assessment questionnaire disability index (HAQ-DI), Cyclic adenosine monophosphate (cAMP), and Cyclic guanosine monophosphate (cGMP). Safety parameters included complete blood count (CBC), liver, kidney and function. Results: A total of 17 individuals with well-controlled RA that complained of fatigue completed the study. The safety profiles were unaffected. Kruskal-Wallis test was used to compare the efficacy at 4 weeks and 8 weeks after treatment, there was no significant difference in efficacy between the groups 4 weeks after treatment (Z=2.914, P=0.233). After 8 weeks of treatment, there was a significant difference in efficacy between the groups (Z=6.753, P=0.034). Further pair comparison showed that the effect of the high-dose 6,000 mg/day group demonstrated greater efficacy than the control group (Z=6.325, P=0.01). Conclusions: Addition of Wisconsin ginseng to conventional therapy in RA with fatigue, thus warrants further investigation to guide the role of Wisconsin ginseng to improve fatigue associated with autoimmune disorders as an adjunct treatment. Abstract: Objective: To explore the anti-influenza pharmacological mechanism of Yinqiao Antipyreti Granules by using network pharmacology. Method: Retrieve the main active ingredients, corresponding targets and target genes of Yinqiao Antipyreti Granules on the website of Chinese Medicine System Pharmacology Technology Platform (TCMSP), obtain the relevant target genes of influenza through the Human Gene Database (Gene Cards), and transfer the drugs The target of the active ingredient is mapped to the influenza target, and the intersection target obtained is the predicted target of Yinqiao Ture granules against influenza. Cytoscape 3.8.0 software was used to construct a drug active ingredient-intersection target network model, and key active ingredients were selected. Use the STRING website to construct an intersection target protein interaction network (PPI) and select key target genes. Enriched analysis of GO and KEGG pathways using Metascape on intersection targets. Results: There are 136 active ingredients in Yinqiao Antipyretic Granules and 131 anti-influenza-related targets. Conclusion: The main components of Yinqiao Antipyretic Granules are quercetin, luteolin, naringenin, formononetin, IL-6, STAT3, MAPK14, CASP3 and other gene targets, to suppress inflammatory response, regulate immunity, reduce the symptoms of influenza, and thus play an anti-influenza therapeutic role.

Abstract: Objective: To introduce and analyze the method and effect of functional acupuncture in treating one case of elderly type 2 diabetic oculomotor nerve palsy. Methods: The main emphasis is on taking the functional position of the affected area, selecting the functional acupuncture area, and choosing the electroacupuncture stimulation dose. Abstract: Background: spinal cord injury is a common traumatic disease. More than 90% of spinal cord injuries are caused by trauma, such as traffic accidents, violence, physical confrontation, sports, etc. Although spinal cord injury is more common and closely related to our life, it is a difficult project to investigate the epidemiological factors of spinal cord injury. In China, there is a lack of national epidemiological analysis of spinal cord injury, and the epidemiological study of spinal cord injury in different cities in China shows different characteristics. Objective: To analyze the clinical characteristics of spinal cord injury and discuss the corresponding treatment. Method: Based on various domestic and foreign literatures on spinal cord injury treatment and rehabilitation, a retrospective analysis was conducted on 137 patients with spinal cord injury admitted to Dalian Port Hospital from 2017 to 2019. Results: The average age of spinal cord injury was 53.92 years. The ratio of male to female patients was 2.83:1; The number of male patients aged 60-69 (30.30%) and female patients aged over 70 (28.57%) was the largest. The injury site of male spinal cord was the most in the neck (54.6%), female the most in the waist (40.6%), followed by the neck (34.4%). In choice of surgical treatment of patients with cervical spine injury patients most (52%), and anterior surgery (43.1%), followed by posterior (35.4%), and thoracic spinal cord injury without anterior surgery patients, most patients (83.3%), posterior surgery patients with lumbar segment damage up to use the posterior surgery (83.3%), a few patients with anterior surgery (6.7%).
