Abstract: Introduction: The use of alpha-blockers facilitates catheter ablation testing (CAT) in patients with benign prostatic hyperplasia (BPH) complicated by complete retention of urine. The aim of our study was to determine the predictive factors for failure of CAT in these patients. Patients and Method: This was a single-centre prospective study conducted at the Urology Department of Aristide Le Dantec Hospital in Dakar from January 1st 2021 December 31st 2021. The parameters studied were: age, digital rectal examination, PSA, CBEU, prostate biopsy, ultrasound and therapeutic results. We tried to determine the factors associated with failure of the urinary catheter removal test. Results: The mean age of the patients was 67.9 +/- 9.2 years (44-93 years). The mean prostate volume was 89.7 ml +/- 42.5. A median lobe was observed in 27.6% of patients. The success rate was higher in the first than in the second CAT (34.2% vs 6%). A higher failure rate was observed (68.9% vs 33.3%) in patients with a PSA > 4 ng/ml (p=0.01). Patients with a prostate volume > 80 ml had a significantly higher failure rate (77.5%) (p<0.01). The failure rate on CAT was higher (p=0.03) in patients who had a median lobe on suprapubic ultrasound (36.2% vs 13.8%). Conclusions: PSA level (threshold 4 ng/ml); prostate volume (80 ml); presence of a median lobe were predictive factors of CAT failure. We found no interest in performing a second CAT.

Abstract: Goal: We aimed to compare the outcome of Mini Percutaneous nephrolithotomy (M-PCNL) performed using our modified supine position with standard prone M-PCNL for renal and proximal ureteric calculi. Materials and Methods: A prospective study between March 2020 to June 2022, including 202 patients undergoing Mini-PCNL was included in this study: 136 patients (160 renal units) underwent in M-PCNL in the modified supine position (Iyyan’s position) and were compared to 66 patients undergoing Mini-PCNL in the prone position. The stone size was assessed by Non Contrast computed tomography scan (NCCT) of kidney, ureter, and bladder (KUB). The surgical outcomes of anesthesia time, surgical time, radiation dose, radiation time, stone free rate, length of stay in hospital and post-operative complications were compared. Chi-square and t-tests were used. Results: Total of 202 patients were registered, 136 patients of supine miniperc PCNL and 66 patients of prone miniperc PCNL. Out of 202 patients 130 patients were male and 72 were female. The mean age (in years) in Supine M-PCNL was 40.62 ± 1.60 while in prone M-PCNL it was 38.61 ± 71. The mean operative time (minutes) was 45.31 ± 72 in Supine M-PCNL and 72.22 ± 16 in Prone M-PCNL. Complete stone clearance was seen in 133 patients in supine M-PCNL versus 60 patients in prone M-PCNL. There were no major complications in both the groups. The mean post-operative hospital stay (days) was 2.6 in supine M-PCNL versus 4.1 in prone PCNL. Stone clearance rate were higher in Supine M-PCNL. Conclusions: Modified supine M-PCNL has significantly less anesthesia time, less surgical time, less radiation time and dose, less hospital stay and higher stone-free rate compared with prone M-PCNL in our series.

Abstract: Objectives: Primary prostatic malignant lymphoma is an exceedingly rare malignancy, accounting for only 0.09% of all prostate neoplasms and 0.1% of all non-Hodgkin's lymphomas. In this report, we present a case of a 70-year-old male patient who presented with sudden-onset hematuria and was ultimately diagnosed with primary lymphoma of the prostate. Methods: The patient presented to a urology outpatient clinic with a 2-week history of gross hematuria but no lower urinary tract symptoms. Digital rectal examination revealed a firm and enlarged prostate, and serum PSA levels were elevated. Ultrasound evaluation, multiparametric prostate magnetic resonance imaging (mp-MRI), and cystoscopy were performed, revealing only a grossly enlarged prostatic median lobe. A systematic transrectal ultrasound (TRUS)-guided prostate biopsy was then performed and confirmed the diagnosis of primary lymphoma of the prostate. Results & Conclusion: This case highlights the importance of considering primary lymphoma of the prostate in patients presenting with macroscopic hematuria and an enlarged prostate, even if mp-MRI does not reveal any other abnormalities. Early diagnosis is crucial in such cases, as it allows for prompt initiation of treatment and may improve clinical outcomes. Given the rarity of this malignancy, clinicians must remain vigilant for its possible presentation and include it in the differential diagnosis when appropriate.

Abstract: Introduction: the objective was to study the place of endoscopic surgery in the management of benign prostatic hyperplasia. Materials and methods: This was a retrospective and prospective descriptive study ranging from July 1, 2020 to November 30, 2021, i.e. 17 months, carried out in the urology department of the University hospital Pr Bocar Sidy Sall at Kati. Were included, all patients who had undergone surgical treatment of benign prostatic hyperplasia by endoscopic route during the study period. The data was entered and analyzed using software: Word 2019, Excel 2019 and SPSS version 25.0. Epi info version 3.53. Result: we collected 70 cases of endoscopic surgery for the management of benign prostatic hyperplasia in 17 months. The age group of 60-70 was the most represented with 42.9% of cases. Lower tract urinary symptoms were the most common reason for consultation, at 68.6%. We performed 21 cases of monopolar TURP, i.e. 30% of cases; 39 cases of bipolar TURP, i.e. in 55.7%; 10 cases of transurethral bipolar vaporization of the prostate or 14.3% of cases. The postoperative course was simple in 92.9% of cases. The probe was removed between D1 and D3 in 62.9% of cases. The average duration of hospitalization was 1.5 days with extremes of 1 and 15 days. Conclusion: Endoscopic surgery for benign prostatic hyperplasia remains the reference treatment in the surgical management of BPH. It is a technique that has proven its effectiveness. Bipolar transurethral vaporization of the prostate, monopolar and bipolar TURP have an important place in the management of BPH in our department.

Abstract: Over a period of three years, two cases of self-amputation of the external genitalia were recorded and managed in our urology division in Souro Sanou University Teaching Hospital. These were male patients and aged 18 years and 32 years respectively. They were irregularly followed up for psychiatric disorders. One was suffering from schizophrenia, the other from paranoid schizophrenia. The first patient self-mutilated with complete sectioning of the penis with a kitchen knife, while the second patient self-mutilated with partial amputation of the penis with exposure of the corpus cavernosa and the left testicle. Managed as an emergency from the onset, the reimplantation of the penile stump could not be carried out due to the lack of an efficient technical expertise with the availability of microsurgery. Hemostasis and refashioning of the amputation stump were performed with definitive urethrostomy for one and penile reconstruction on urethral catheter and directed healing for the other. Postoperatively the two patients were able to urinate through the urethrostomy but the cosmetic appearance of the external genitalia was unsatisfactory. Although rare, self-mutilations are dramatic and serious accidents that can be life-threatening and functionally disabling for patients. Their management must be multidisciplinary, involving urological surgeons, intensive care physicians, emergency physicians and psychiatrists with the treatment of the underlying psychiatric disorder.

Abstract: Introduction: Emphysematous pyelonephritis is a rare and serious medical-surgical condition. Emergency nephrectomy is the standard treatment for forms that respond poorly to medical therapy, in conjunction with drainage of the upper excretory tract. Case report: This is a case of emphysematous pyelonephritis in a 54-year-old female patient who had presented with acute pyelonephritis, left renal colic and hyperactivity syndrome. A non-injected abdominopelvic CT scan revealed a globular, dedifferentiated left kidney with a diffuse aortic component. The right kidney was unremarkable. In intensive care, despite placement of a double JJ tube and antibiotic therapy with amikacin and cefotaxime, the patient's hemodynamic and neurological status continued to deteriorate, necessitating intubation. After 24 hours of well-managed treatment with no improvement, nephrectomy was performed in the setting of emphysematous destruction of the left kidney and persistent latent shock. Based on this observation and a review of the literature, we describe the clinical and radiological features, as well as the various diagnostic and therapeutic means of this condition. Conclusion: the diagnosis of emphysematous pyelonephritis should be suspected in the following cases: the presence of severe acute pyelonephritis, with no improvement in clinical condition despite well-conducted medical treatment or percutaneous drainage, or associated with signs of severity, particularly in diabetics, radical treatment is envisaged after failure of drug therapy.

Abstract: Introduction: The imaging assessment of the male genital tract in the diagnostic evaluation of infertile men is based on scrotal Doppler ultrasound. The objective of this study was to determine the place of scrotal ultrasound in the diagnosis of male infertility. Methodology: This was a descriptive retrospective study lasting 3 years, from January 1st, 2019 to December 31, 2021. It focused on patients referred to the medical imaging, exploration and diagnosis in Conakry for a scrotal ultrasound in the context of couple infertility. Results: 241 patients were seen for scrotal ultrasound for infertility. An ultrasound abnormality was found in 181 patients (75.1%). The average age of the patients was 41.5 years with extremes of 26 and 67 years. The localization of abnormalities identified on ultrasound was predominant in the spermatic cord with 127 cases (70.2%), followed by testicular, epididymal and scrotal affections. There was an association of abnormalities in 40.9% of cases (n=74). Varicocele was the main ultrasound anomaly of the spermatic cord, 66.8% (121 cases) followed by testicular hypotrophy and epididymal cysts. Conclusion: Scrotal ultrasound occupies an important place in the diagnostic process of male infertility.

Abstract: Background: Disorders of sex development (DSD) are a group of congenital conditions associated with anomalous development of internal and external genital organs. Ovotesticular disorders of sexual development formerly called true hermaphroditism are the rarest of all types of disorders of sexual development. It refers to individuals who have both ovarian and testicular tissues in the same or contralateral gonad, regardless of their karyotype. Case presentation: Ovotesticular DSDs are usually diagnosed in the pediatric age group. We reported a rare case of ovotesticular DSD in an adult, originally assigned as male gender. He was referred to our service with the complaint of difficulty with sexual intercourse and genital ambiguity. On examination, he was found to have gynecomastia, a female pelvic, a left testicle, micropenis, and proximal hypospadias. A total abdominal hysterectomy with penile reconstruction and left orchidopexy was performed. Hypospadias repair was planned to be done later. We aim to highlight the challenges urologists practising in a poor-resource community face in diagnosing and managing of these cases. Conclusions: Ovotesticular DSD is rare. Diagnosis is usually made clinically supported by laboratory, imaging and histological evaluation. Treatment consists of multidisciplinary management including urologists, gynaecologists, endocrinologists, paediatricians, and psychiatrists. It is important to involve the patient and their family in shared decision-making before surgery or gender assignment.

Abstract: Introduction: scrotal ou testicular trauma is often associated with low mortality with a significant risk of infertility in men. The aims of our study was to analyze the epidemiological, clinical and therapeutic aspects of bursal traumas. Patients and methods: retrospective descriptive study of 20 patients followed for bursal trauma from January 2007 to December 2021 at a university hospital in Senegal. Parameters studied: age, reason for consultation, consultation delay, nature of trauma, lesion assessment, treatment carried out and therapeutic results. The primary endpoint was the incidence of scrotal or testicular lesions. Results: The mean age of the patients was 32.7 years. Scrotal pain (n=19) was the most frequent reason for consultation. The average consultation time was 45 min. There was closed trauma in 14 patients and open trauma in 6. The main cause of closed trauma was road traffic accidents (n=9). Ultrasonography of the bursa was performed in all patients with closed bursal trauma, and hematocele (n=8) was the most frequent lesion revealed by this examination. Fifteen patients underwent surgery and 5 had conservative treatment. Evacuation of a hematocele (n=9) was the most frequently performed surgical procedure. Testicular lesions were the most common intraoperatively (n=10), with a predominance of AAST grade V lesions. Most of the post-operative course was straightforward. Three patients had oligoasthenozoospermia on the post-traumatic spermogram. Conclusion: bursal traumas was a rare emergency involving young adults. Few complications were noted after treatment.
