Mid-Term Efficacy of Surgical Treatment for Calcaneal Fracture Combined with Die-Punch Bone Block

Abstract: To discuss the surgical efficacy and experience of calcaneal fracture combined with die-punch bone block. Open reduction and internal fixation for 27 patients (27 feet) with calcaneal fracture combined with die-punch bone block were performed, the die-punch bone blocks were untreated in earlier stage, calcaneal anatomical recovery and function were evaluated during follow-up in one year after surgery. The follow-up results for the 27 patients in one year after the surgeries show the postoperative width of intraoperative die-punch bone block treatment group was significantly better than that of non-treatment group (p<0.05), the postoperative functional scores of treatment group were superior to those of non-treatment group (p<0.05), and there were no significant differences in preoperative and postoperative calcaneal height, length, Bohler angle and Gissane angle between treatment group and non-treatment group (p>0.05). According to 100-point criteria of Tieliang Zhang, there were 1 excellent case, 2 good cases, 4 medium cases and 2 poor cases in non-treatment group, and there were 5 excellent cases, 10 good cases, 2 medium cases and 1 poor case in treatment group; the scores of the treatment group were better than those of the untreated group (p<0.05). For intra-articular calcaneal fractures combined with die-punch bone block, it is necessary to pay attention to the surgical anatomical reduction. The steps of articular surface should be controlled within 1mm. The treatment for calcaneal fracture combined with die-punch bone block must be performed by open reduction and internal fixation, and meanwhile the treatment for die-punch bone block should be emphasized, in order to reduce the occurrence of dysfunction during recovery.

Abstract: Background: Many options are available for treatment sacrococcygeal pilonidal sinus disease either surgical or medical, but surgery is preferred. There are many techniques for surgical treatment but there is controversy about the best technique. Until now there is no single procedure is superior in all aspects. The aim of this study was to evaluate modified Bascom Clift lift procedure in management of. Patients and methods: From April 2014 to July 2018, 150 patients with pilonidal sinus were treated with Clift lift procedure and followed. Data collected tabulated and analyzed included complications, recurrence, post-operative hospital stay, days-off work, postoperative aesthetic appearance and patient's satisfactions. Results: There were 150 patients, 129 male and 21 females with M:F ratio. Their ages ranges from 16 to 45 year, with mean age 22.55± 8.5 year. Fifteen patients (10%) had previous operation for pilonidal sinus (recurrent). After operation all patient were mobilized at first day, mean postoperative pain VAS score were about 2(1-3) and discharged from hospital at 2nd day and were returned to daily activities after mean 7 day (6-12 days), return to work at mean (12.5) days with good healing within 10 days, postoperative wound infection had occurred only in three patients (2%). Patients satisfaction score were very good. No recurrence was observed during follow up period. Conclusion: Modified Bascom natal cleft lift is associated with good outcome, it is simple procedure and easy to learn, with early mobilization, short hospital stay early return to daily activities and work, and good patients satisfaction and to somewhat accepted post-operative scar. More studies with larger group of patients are needed especially in recurrent cases to come to consensus.

Abstract: Background: Pneumothorax and pulmonary air leakage (PAL) are common emergency and complication in the clinical department of thoracic surgery, the treatment measures of air leakage including observation of conservative, needle aspiration, chest tube drainage, surgical operation and pleurodesis. In the selection of treatment methods, there is not a unified and feasible standard to choose a reasonable, effective and economic therapeutic regimen. Inapposite treatment will prolong hospitalization days, increase cost, even may lead to a death. Therefore, Sorting out and summarizing the therapies is vitally important for clinical conduction. Method: We retrieved about 80 papers in Pubmed, Science Direct, and China How Net while searched "Pneumothorax" or "longer air leakage" And "treatment" as keywords or title in recent ten years, 40 reports have been reviewed. We summarize the selection and development of the treatments for pneumothorax and PAL. Conclusions: According to the cause, age, urgency and other symptoms, the clinician should take different treatment. The minimally invasive closed drainage will become a preference for the pneumothorax needing drainage, it may replace the traditional tube closed drainage. VATS will become the preferred treatment of pneumothorax with bubble. Autologous blood is a facilitate getting adhesive agents, its procedure is easy and safe with few complications, and it is also an economical method with a high success rate. However, it still needs further studies to taking the economical, effective, appropriate treatment for a special pneumothorax and postoperative lung air leak.

Abstract: Objective The aim of this study was to evaluate the beneficial effects and applicability of a half-thickness continuous suture outside the pectinate muscles used in the right atrial incision for adult patients undergoing cardiac surgery by cardiopulmonary bypass. Methods A total of 1040 consecutive adult patients undergoing cardiac surgery through a right atrium incision during the period of January 2010 to June 2014 were randomly allocated to the experimental group (n=522) and the control group (n=518). In the experimental group, a half-thickness continuous suture outside the pectinate muscles was used to close the right atrium incision, while the traditional method of a continuous full-thickness everting suture was used in the control group. The occurrences of bleeding spots in the right atrium sutures were recorded prior to the pericardial closure procedure. Results The occurrence rates of intra-operative bleeding from the atrial incision site for the experimental group were significantly lower than that in the control group (46 vs. 253 patients needed re-stitch procedures for the right atrial incision respectively, p=0.001). In the control group, two cases of re-exploration due to post-operative bleeding in the right atrial incision occurred, while none occurred in the experimental group. Conclusions The half-thickness continuous suture outside the pectinate muscles for closing right atrium incisions could effectively reduce the need for re-stitch procedures, as well as minimize the risks of bleedings for patients undergoing cardiac surgeries.

Abstract: [Objective] Explore the effectiveness and pracaticability of the lateral tibia bone transport technology for the treatment of thromboangiitis obliterans of lower limbs [Methods] 52 patients with thromboangiitis obliterans by lateral tibia bone transport operation. On the fifth day after the surgery, the tibial bone mass was transversely transported under the action of external stent, after carry out for 21mm, hold for 3 days, carry back again, finish the bone transport in 50 days after surgery, then the external stents were removed, and then evaluate the effectiveness by angiography, limbs survived and follow-up [Results] 45 of 52 cases obtained satisfactory curative effect, limb ischemia necrosis were completely improved. through 2 ~ 3 years follow-up time. There are still severe lower limb ischemia necrosis cases, 7 cases of postoperative lower limb amputation. The total effective rate was 87%. [Conclusion] Lateral tibia bone handling technology can be effective treatment of thromboangiitis obliterans of lower limbs, the surgical operation is simple, safe and effective, the method is worthy of clinical promotion.

Abstract: The present review aims to analyze the current data available on the different endoscopic surgical techniques described for Isthmocele treatment. A semantic review of literature was made of all english language publications on databases Pubmed and Google Scholar following a Mesh and key word searching. The studies were finally selected by one author according to the aim of this review. The isthmocele, defined as a defect of the anterior wall of the uterine cervical canal at the site of a previous cesarean section scar, is a prevalent condition in women submitted to gynecological endoscopic procedures. Epidemiological studies report a 1.9% of cesarean scar defects, rising in the last decades, and it is clear that the repair carrie benefits in terms of symptom control and sub-fertility. The minimally invasive approach is the gold standard due the inherent benefits in quality and magnification of image, less postoperative pain, hospital stay and minor complications rate. It can be treated either by laparoscopy or hysteroscopy according to the presentation of the disease and the preference of the surgical team, since both have the same efficacy in symptoms improvement (59% to 90%). Laparoscopy is usually indicated when overlying myometrial mantle is less than 3 mm, and allows an increase in postoperative myometrial streght. Different surgical techniques have been described and supposedly, there is no significant difference among them. Large analytical prospective experimental studies are required to confirm the results of this data and to investigate as to whether there is a better laparoscopic technique.
